THE EFFECTS OF AMOXICILLIN WITH OR WITHOUT CLAVULANIC ACID ON THE POSTOPERATIVE COMPLAINTS AFTER THIRD MOLAR SURGERY: A RETROSPECTIVE CHART ANALYSIS

Öz:
Amaç: Hasta kayıtlarının incelenmesine dayanan bu geriye dönük çalışmanın amacı; alt çene gömük üçüncü büyük azı dişinin cerrahi çekimi sonrasında ağız yoluyla sadece amoksisilin ve amoksisilin-klavulonik asit içeren antibiyotiklerin verilmesinin girişim sonrasında meydana gelen komplikasyonların sıklığı ve hasta konforu üzerindeki etkilerini değerlendirmektir.Gereç ve Yöntem: 2014 yılının Ekim ayı ile 2015 yılının Aralık ayı arasında cerrahi olarak alt üçüncü büyük azı dişleri çekilen 453 hastanın kayıtları incelendi. Bu çalışmaya kriterlere uyan, dişleri mesioanguler pozisyonda konumlanmış ve aynı antiinflamatuar ilacı kullanmış olan toplam 62 hasta dâhil edildi. Bu hastaların arasında 32 kişinin girişim sonrasında 5 gün boyunca, her sekiz saatte bir oral yoldan 500 mg amoksisilintrihidrat (A grubu), 30 hastanın ise her sekiz saatte bir oral yoldan 500 mg amoksisilintrihidrat + 125 mg potasyum klavulonat (AC grubu) kullandığı belirlendi. Girişim sonrasında hissedilen ağrı seviyeleri, şişlik, trismus varlığı, alveolar osteit sıklığı ve yaşam kalitesi ölçeği (YKÖ) skorları ile ilgili veriler hasta dosyalarından elde edilerek istatistiksel olarak karşılaştırıldı.Bulgular: Çalışmanın değişkenleri incelendiğinde; gruplar arasında ağrı, şişlik, trismus ve YKÖ skorları bakımından istatistiksel olarak anlamlı bir farklılık olmadığı saptandı. A grubunda alveolar osteit sıklığının %1.6 olduğu bulunmasına karşılık çalışma grupları arasında anlamlı bir farklılık izlenmedi.Sonuç: Hasta kayıtları temel alınarak yapılan bu geriye dönük çalışmanın sınırlılıkları dahilinde, alt üçüncü büyük azı dişinin cerrahi çekimi sonrasında sadece amoksisilin ya da amoksisilin-klavulonik asit içeren antibiyotiklerin kullanımının hasta konforu üzerinde benzer etkileri olduğu söylenebilir

Öz:
Purpose: The aim of this chart-based retrospectivestudy was to evaluate the effects of orally administeredamoxicillin alone or amoxicillin combined with clavulanicacid on the frequency of post-operative complications andpatients’ comfort after mandibular third molar surgery.Materials and Methods: The records of patients who hadundergone lower third molar surgery between October2014 and December 2015 were examined. A total of 62patients who had fully impacted teeth in mesioangularposition and who had been prescribed same type and dose ofanti-inflammatory drug were included in this study. Amongthem, 32 subjects were found to have been prescribed500 mg amoxicillin trihydrate orally every 8 h for 5 days(Group A) and 30 patients 500 mg amoxicillin trihydrateplus 125 mg potassium clavulanate orally every 8 h for5 days postoperatively (Group AC). Post-operative painlevels, swelling, presence of trismus, frequency of alveolarosteitis and quality of life (QoL) scores were gatheredfrom patients’ charts and were statistically compared.Results: Analysis of the variables showed that therewere no significant differences between the GroupsA and AC regarding pain levels, swelling, trismusand QoL scores. The frequency of alveolar osteitiswas found to be 1.6% in the Group A, however, nosignificant difference was observed among study groups.Conclusion: Within the limitations of this retrospective chartreview, it can be stated that amoxicillin and amoxicillinwith clavulanic acid might provide similar outcome interms of patient comfort following third molar surgery.
