Diyabetik Maküla Ödeminde Seröz Maküla Dekolmanı ve Glikolize Hemoglobin ile İlişkisi*

Öz:
Amaç: Diyabetik maküla ödemi (DMÖ) olan hastalarda seröz maküla dekolmanı (SMD) sıklığını ve glikolize hemoglobin (HbA1c) ile ilişkisini araştırmak. Gereç ve Yöntem: Kliniğimiz retina biriminde DMÖ nedeniyle takip edilen 124 hastanın 124 gözü retrospektif olarak incelendi. Optik koherens tomografi (OKT) bulgularına göre sadece DMÖ bulunan olgular Grup 1; DMÖ'ye eşlik eden SMD bulunan olgular Grup 2 olarak tanımlandı. Gruplar yaş, diyabet süresi, hipertansiyon (HT) varlığı, HbA1c düzeyi, en iyi düzeltilmiş görme keskinliği (EİDGK) ve merkezi maküla kalınlığı (MMK) açısından karşılaştırıldı. Bulgular: Hastaların 66'sı erkek (%53), 58'i kadın (%47) olup yaş ortalaması 63.4±7.5 yıl idi. Çalışma kapsamına alınan 124 hastanın 57'sinde (%45.9) SMD mevcuttu. Gruplar arasında yaş, diyabet süresi, HT varlığı açısından istatistiksel olarak anlamlı fark bulunmadı. Ortalama HbA1c düzeyi Grup 1'de 8.6±0.9 (6.1-11.3) mg/dl iken; Grup 2'de 9.8±1.32 (6.9-14.2) mg/dl idi ve aradaki fark istatistiksel olarak anlamlı idi (p<0.001). Ortalama EİDGK Grup 1'de 0.54±0.18 LogMAR (1-0.30) iken; Grup 2'de 0.64±0.23 LogMAR (1.30-0.40) idi ve istatistiksel analizde iki grup arasındaki fark anlamlı idi (p<0.001). Ortalama MMK Grup 1'de 398±72.4 µm; Grup 2'de ise 434.6±66.8 µm idi ve aradaki fark istatistiksel olarak anlamlı idi (p<0.001). Sonuç: Çalışmamız sonucunda SMD'nin DMÖ'lü hastalarda oldukça sık gözlendiğini ve SMD varlığının yüksek HbA1c düzeyleri ile ilişkisi olabileceğini saptadık.

Relationship Between Serous Retinal Detachment and Serum Glycosylated Hemoglobin Levels in Diabetic Macular Edema

Öz:
Purpose: To investigate the frequency of serous retinal detachment (SRD) in diabetic macular edema (DME) and relationship with serum glycosylated hemoglobin (HbA1c) levels. Material and Methods: 124 eyes of 124 patients with DME were investigated retrospectively. Cases with only DME was defined as group 1 and cases with DME and SRD was defined as group 2 due to optic coherence tomography (OCT) findings. Age, duration of diabetes, hypertension, HbA1c levels, best corrected visual acuity (BCVA) and central macular thickness (CMT) were compared between the groups. Results: The mean age of 66 male (%53) and 58 female (%47) patients was 63.4±7.5 years. SRD was present in 57 of 124 patients (45.9%). Age, duration of diabetes and presence of hypertension were statistically similar between the groups. Mean HbA1c level was 8.6±0.9 (6.111.3) mg/dl in group 1 and 9.8±1.32 (6.9-14.2) mg/dl in group 2 and the difference was statistically significant (p<0.001). Mean BCVA was 0.54±0.18 LogMAR (1-0.30) in group 1 and 0.64±0.23 LogMAR (1.30-0.40) in group 2 and the difference was statistically significant (p<0.001). Mean CMT was 398±72.4 µm in group 1 and 434.6±66.8 µm in group 2 and the difference was statistically significant (p<0.001). Conclusion: In our study, SRD was found to be common in patients with DME and higher HbA1c levels seemed to be related with presence of SRD.
