The 2019 coronavirus disease epidemic that started in China has developed into a global health threat, and great efforts are being made to stop the spread of this pandemic worldwide. In the present study, we inquired into the association between the global epidemic and depression and anxiety in school-age children. Methods: In this study, school-age children were assessed via an online survey in terms of depression and anxiety symptoms and several aspects relating to the COVID-19 pandemic. The depression level was assessed with The Child Depression Inventory. The anxiety levels were evaluated with the Childhood Anxiety Sensitivity Index and the State-Trait Anxiety Inventory for Children. Results: A critical result of our study is the percentage of children and adolescents who acquired scores above 19 on the Child Depression Inventory. This cut-off point is considered significant, as it reflects a pathological level of depression. Of the sample population, 316 (41.79%) had Child Depression Inventory scores that exceeded this value. These data point to the fact that school-age children are affected approximately 15 times more severely than normal by the COVID-19 pandemic. The regression analysis showed significant effects of anxiety, age, and knowledge of COVID-19 on the Child Depression Inventory scores. Conclusion: The present study demonstrated the effects of the COVID-19 pandemic on the depression and anxiety levels in school-age children. Our results predict that there is an urgent need for psychosocial support for school-age children. Moreover, there will be a considerable increase in the incidence of psychiatric disorders among school-age children if the psychological aspects of the pandemic are ignored

Amaç: Tümör nekroze edici faktör alfa (TNF-α), akut pankreatit patogenezinde ve komplikasyonların gelişiminde anahtar role sahiptir. Çalışmanın amacı, ratlarda oluşturulmuş şiddetli akut nekrotizan pankreatit modelinde TNF-α inhibitörü olan Adalimumab’ın (ADA) etkinliğinin araştırılmasıdır. Yöntem: 70 adet Wistar albino cinsi dişi rat Grup 1: SHAM laparotomi, Grup 2: Modifiye SHOP modeli ile şiddetli nekrotizan pankreatit (MS-SNP), Grup 3: MS-SNP ve intraperitoneal (İP) salin, Grup 4: ADA 2 mg/kg İP + MS-SNP, Grup 5: ADA 4 mg/kg İP + MS-SNP, Grup 6: ADA 4 mg/kg İP ve Grup 7: ADA 2 mg/kg İP olarak gruplandırıldı. MS-SNP; ortak safra ve pankreas kanalınının duodenuma girmeden önce bağlanması ve 50 μg/kg cerulein İP verilmesiyle oluşturuldu. Ratlar AP oluşturulmasını takiben 24. saatte sakrifiye edilerek; serum pankreatik amilaz ve pankreas dokusunda ödem, nekroz, inflamasyon, yağ nekrozu ve kanama parametreleri değerlendirildi. Bulgular: Deneysel ADA etkinliğini araştırdığımız Grup 4 ve 5 ile Grup 2 kıyaslandığında pankreatik ödem, inflamasyon, nekroz, kanama, yağ nekrozu ve total patoloji skorları düşük tespit edilmiş, Grup 5’te tüm parametreler açısından anlamlı farklılık elde edilmiştir (P< ,05). Grup 4’te ise yalnızca pankreatik ödem, yağ nekrozu ve total patoloji skorlarındaki düşüklük anlamlı bulunmuştur. Pankreatik amilaz değerlerinde anlamlı düşüş saptanmamıştır. ADA’nın uygulanan dozlarda tek başına pankreatit yapmadığı da gösterilmiştir. Sonuç: ADA uygulanan grupta pankreatik ödem, nekroz ve kanama gibi histopatolojik değişikliklerin oluşması engellenmiş ve histopa- tolojik skorlar ADA uygulanmayan gruplara göre anlamlı derecede düşük saptanmıştır. Gelecekte ADA’nın AP tedavisinde kullanımıyla ilgili ümit verici sonuçlar elde edilmekle birlikte bu amaca yönelik farklı modeller ve farklı ADA dozlarıyla benzer çalışmalara ihtiyaç vardır

Objective: This study aimed to investigate the effect of phototherapy on serum electrolyte levels in neonates admitted to neonatal intensive care unit with the diagnosis of neonatal hyperbilirubinemia.Methods: Demographic profiles, laboratory findings, and electrolyte levels before phototherapy and 48-72 hours after phototherapy of260 patients hospitalized with neonatal hyperbilirubinemia between June 2016 and June 2018 were compared in this retrospective study.Results: The mean sodium level significantly decreased from 140.5 mg/dL to 139.1 mg/dL after phototherapy. The decrease in potassiumlevel was not statistically significant. The mean calcium level significantly decreased from 9.98 mg/dL to 9.55 mg/dL. The mean chloridelevel before phototherapy significantly decreased from 106.4 mmol/L to 105.9 mmol/L after phototherapy.Conclusion: The level of serum electrolytes in newborns may change with phototherapy. Serum sodium, chloride, and calcium levelsmay decrease after phototherapy. For prevention of possible adverse effects, the changes in electrolyte levels should be considered in theclinical course of newborns receiving phototherapy and appropriate fluid-electrolyte treatments should be given.

Amaç: Bu çalışmada, portal hipertansiyonlu çocuk hastalarda, portal hipertansiyon ve tedavi edici girişimlerin gastroözofageal reflühastalığı ile ilişkisini araştırmak hedeflendi.Gereç ve Yöntemler: Çalışmaya 30 portal hipertansiyonlu çocuk hasta alındı. Hastalar üç ana gruba ayrıldı;Grup 1: Endoskopik skleroterapi uygulanmamış olgular.Grup 2: Endoskopik skleroterapi programında olan olgular.Grup 3: Devaskülarizasyon ameliyatı yapılan olgular.Hastaların gastroözofageal reflü şikayetleri, endoskopi ve 24 saatlik pH monitorizasyonu bulguları kaydedildi. İstatistiksel inceleme Fisher-Exact yöntemi ile yapıldı.Bulgular: Çalışmada portal hipertansiyonlu olgularda, üç incelemede de, gastroözofageal reflü toplumdakine oranla belirgin olarak fazlagörüldü. Klinik reflü göz önüne alındığında gruplar arasında anlamlı fark bulunmadı. Endoskopi bulguları göz önüne alındığında Grup 1 ve3’te Grup 2’ye göre anlamlı olarak fazla saptandı. Buna karşılık 24 saatlik pH metre incelemelerinde, gruplar arası anlamlı fark bulunmadı.Sonuç: Portal hipertansif olgulardaki gastroözofageal reflü araştırmasında tek bir yöntem yeterli değildir. Klinik belirti ve bulgular, endoskopik muayene ve pH monitorizasyonu birlikte değerlendirilmelidir. Yüksek reflü oranı nedeni ile tüm portal hipertansif çocuklarıntedavi altına alınması akılcı bir yaklaşım olarak görülmektedir.

Systemic lupus erythematosus may present with different clinical signs such as fatigue, fever, dermatitis, photosensitivity, alopecia,arthritis, serositis, hematological abnormalities, mucosal ulcerations, Raynaud phenomenon, neurological disease, and glomerulonephritis. Rarely, kidney involvement with the absence of antinuclear antibody (ANA) positivity may be the first and only finding insystemic lupus erythematosus. Since renal involvement affects prognosis negatively, delays in diagnosis and treatment can cause anincrease in mortality and morbidity. In this article, we presented an interesting case of systemic lupus erythematosus, who applied tothe family medicine outpatient clinic with periorbital edema in whom we considered renal involvement and determined histopathological findings that were compatible with lupus nephritis despite the absence of ANA positivity

Ligamentum falciforme separates liver from the left lateral and medial segments along the umbilical fissure and suspends the liveron the anterior wall of the abdomen. The ligamentum teres hepatis is a remnant of the obliterated umbilical vein, which lies on thefree edge of the falciform ligament. Moreover, it contains paraumbilical venules, muscle fibers, various fatty tissue amounts, andtwo mesothelial layers. Pathologies of the falciform ligament are considerably rare. The ligament is usually secondarily affected bysurrounding inflammatory diseases. Because of its rarity, necrosis, or abscess of the falciform ligament is usually misdiagnosed as anabdominal wall abscess, and inappropriate treatment leads to the lesion’s persistence and complications. Surgical resection is thetherapy of choice. The main purpose is to remind that respiratory problems can be the primary complaint of the patient who appliesto the emergency room with falciform necrosis secondary to acute pancreatitis.

Objective: Breast is one of the secondary sex characters defining gender. It would be an appropriate approach for breast surgeons toevaluate the breast aesthetically when performing aesthetic and reconstructive breast procedures.Methods: Four different women with different breast sizes and shapes were selected. A questionnaire form was prepared with the photostaken from the front and right obliques of these four different women. The participants were asked to evaluate the symmetry, shape, andsize of the breasts by looking at the photos and to determine and interpret the attractiveness score.Results: The participants were 430 men and 570 women. The B cup breast received the highest attractiveness score among the modelsin all the groups. Sagging has been the most commented and criticized parameter.Conclusion: In this study, we aimed to evaluate the aesthetic perspective of breast in our society and believe that it is beneficial in termsof understanding the patient’s expectations, guiding her correctly, and improving what we can do as surgeons.

Objective: Health-related quality of life (HRQOL) is a concept that assesses the effects of physical symptoms, functional status, andcurrent illness on psychological and social functioning. In this study, we aimed to examine the quality of life in children and adolescentswith familial Mediterranean fever (FMF) according to personal and disease-related factors.Methods: A total of 257 children and adolescents with FMF aged 8–18 years were included in the study. The Juvenile Auto-inflammatory Disease Multidimensional Assessment Report was used for assessment of functional ability (self-report scale), pain (visual analog scale - VAS), therapeutic compliance, and HRQOL (physical, social, school, and emotional status). An increase in all subscale scores indicated a worsening.Results: The mean age of the patients was 12.4±2.63 years, and 132 (51.4%) of them were women. In adolescents, HRQOL-general,-social, -school, and -emotional scores were higher than children (p<0.001 for all). Also, all HRQOL subscale scores of females weresignificantly higher than males (p<0.05 for all). Patients who had an attack in the last month had higher functional ability, pain, and allHRQOL subscale scores. However, patients with and without exertional leg pain had similar results in terms of all HRQOL subscales. TheVAS pain score was positively correlated with HRQOL-general, -physical, -school, and -emotional scores. The therapeutic compliancescores were positively weakly correlated with all HRQOL subscales other than HRQOL-physical.Conclusion: This study showed that the quality of life was worse in the adolescent age group, female gender, and the presence of attacks

Herein, we report a patient with dementia, parkinsonism, and motor neuron disease owing to possible central nervous system involvement of Whipple disease. Although co-existence of dementia, parkinsonism, and second-order motor neuron involvement may be seenin some degenerative disease complexes, excluding a reversible condition is a necessity. The critical issue in diagnosing our patient wasthe presence of gastroparesis and arthritis in history associated with progressive supranuclear palsy-like clinical features. Impairment inany domain of cognition may be observed in Whipple disease. Our patient had prominent attention and memory deficits. To our knowledge, this is the first case of generalized lower motor neuron involvement with cognitive deficits and parkinsonism in Whipple disease.
