Spontan preterm doğum ve preterm prematür membran rüptürü olgularının değerlendirilmesi

Öz:
AMAÇ: Kliniğimizdeki spontan preterm doğum (SPD) ve preterm prematür membran rüptür (PPROM) olgularını karşılaştırmak. Planlama: Retrospektif çalışma. Ortam: KSÜ Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniği, Kahramanmaraş Hastalar: Erken doğum yapan hastalar. Girişim: Yok. Değerlendirme parametreleri: SPD ve PPROM’lu olguların demografik özellikleri, klinik takip, tedavi ve gebelik sonuçları; preterm eylem için risk faktörleri. SONUÇ: 2006 yılında kliniğimizde 8 aylık dönemde toplam 596 doğumun 104’ ü (% 17.4) preterm doğum idi. Yüzdört olgunun 46' sı (% 44.2) SPD (grup 1), 42’ si (% 40.3) PPROM (grup 2) ve 16’ sı (% 15.3) obstetrik nedenlerle yapılan preterm doğum olgusu idi. SPD’ li ve PPROM’ lu 88 preterm doğum olgusu retrospektif olarak incelendi. Her iki grup olguların demografik özellikleri benzerdi. Preterm eylem risk faktörlerinden ‘önceki gebeliklerinde PPROM öyküsü’ olan olgu sayıları PPROM grubunda 19 (% 45) iken SPD grubunda 5 (% 11) idi (p< 0.05). Bunun dışındaki diğer tüm risk faktörleri arasında istatistiksel olarak anlamlı fark bulunamadı. Klinik takip, tedavi ve gebelik sonuçlarına ait parametrelerden doğumdaki gebelik haftası, 1 ve 5. dakikadaki Apgar skorları ve bebek doğum ağırlığı da benzerdi (p> 0.05).YORUM: SPD ve PPROM’lu olgulara ait risk faktörlerinin aynı; klinik özellikler ve gebelik sonuçlarının benzer olduğu gözlendi. Bu sonuçlar, bu iki durumun aynı hastalığın farklı klinik yansımaları olduğunu desteklemektedir.

Evaluation of the cases with spontaneous preterm delivery and preterm premature rupture of membrane

Öz:
OBJECTIVE: To compare cases of Spontaneous Preterm Delivery (SPD) and Preterm Premature Rupture of Membrane (PPROM). Design: A retrospective study. Setting: KSU Medical Faculty Department of Obstetric and Gynecology, Kahramanmaraş. Patients: Preterm delivered pregnant women. Interventions: None. Main Outcome Measures: Demographic features, clinical follow up, management and obstetric outcomes in SPD and PPROM cases; risk factors for preterm delivery.RESULTS: One hundred and four (17.4%) preterm births were detected among a total of 596 delivery for a duration of 8 months in 2006. 46 of 104 (44.2%) preterm birth cases were SPD (Group 1), 42 (40.3%) were PPROM (Group 2) and the rest 16 cases (15.3%) were preterm birth of obstetrical causes. Eighty eight preterm birth cases with SPD and PPROM were analyzed retrospectively. Demographic features of both groups were similar. &#8220;PPROM history in previous pregnancy&#8221;, one of the risk factors for preterm labour, was positive in 19 (45%) of PPROM cases and in 5 (11%) of SPD cases (p< 0.05). No significant difference in other risk factors was detected. Gestational age at birth, Apgar scores in 1- and 5-min and birthweight, clinical follow-up parameters, management, and obstetric outcomes, were similar as well (p> 0.05). CONCLUSIONS: SPD and PPROM cases had the same risk factors and clinical features and obstetric outcomes were similar. Our findings support the opinion that these two situations are different reflections of the same disease
