Denge fonksiyonunun değerlendirilmesi

Öz:
Denge, koordinasyon kavramı içinde değerlendirilmektedir ve basitçe destek yüzeyi üzerinde vücut ağırlık merkezini koruma yeteneği olarak tanımlanır. Koordinasyon ise amaca uygun, düzgün, kontrollü hareketler yapabilme yeteneğidir. İnce motor yeteneklerin kullanılmasında, mesleki aktivitelerin gerçekleştirilmesinde, yürüme, koşma, atlama gibi günlük yaşamla ilgili basit ve yardımcı aktiviteleri yapmada motor koordinasyon gereklidir. Koordine hareketler, iyi bir denge ve postür fonksiyonu ile birlikte sinerjistik ve resiprokal kas aktivitelerinin doğru sıralama ve zamanlamasını gerektirir. Erekt postürün sağlanması ve aktiviteler sırasında dengenin sürdürülebilmesi için, oldukça karmaşık nöromüsküler mekanizmalar gereklidir. Bu mekanizma, çeşitli duysal kaynaklardan (proprioseptif, görsel, vestibuler) vücudun uzaydaki konumu, yönelimi ve hareketleri ile ilgili bilgi elde eden ve bu bilgiyi kütle merkezini destek merkezinde tutma konusunda uygun bir motor yanıt üretmek için kullanan bir sinir kas etkileşiminden oluşmaktadır. Birçok sistemin katıldığı, oldukça karmaşık bir fonksiyon olan dengenin değerlendirilmesinde; duyu, motor ve biyomekanik faktörler göz önünde bulundurulmalıdır. İncelemeler problemin nedenleri ve derecesi hakkında fikir verecek nitelikte çok boyutlu olmalıdır. Bu amaçla denge fonksiyonunun değerlendirilmesine yönelik basitten karmaşık tekniklere kadar birçok klinik ve laboratuvar değerlendirme yöntemi geliştirilmiştir. Denge bozukluğu, duyusal ve motor kontrol sistemini etkileyen birçok nedenle ortaya çıkabilir ve fonksiyonel kapasiteyi, bağımsız yaşam ve yaşam kalitesini olumsuz yönde etkileyebilir. Doğru bir değerlendirme ile tedaviye yönelik tıbbi rehabilitasyon ve cerrahi müdahele planları kolaylaşır. Bu derleme makalede klinik uygulamalarda ve bilimsel çalışmalarda denge fonksiyonunun değerlendirilmesinde kullanılan klinik ve laboratuvar değerlendirme yöntemleri üzerinde durulmaktadır.

The evaluation of the balance function

The balance is evaluated within the concept of the coordination and it is simply defined as the ability to keep the center of gravity of the body on the support surface. The coordination is the ability to make relevant, proper and controlled moves. Motor coordination is required in use of fine motor skills, in realisation of professional activities and performing basic and assistive activities associated with daily life such walking, running and jumping. Coordinated moves require a proper sequence and timing of the sinergistic and reciprocal mucle activities beside a good balance and posture function. Very complicated neuromuscular mechanisms are necessary to obtain the erect posture and to maintain the balance during activities. This mechanism includes a neuromuscular interaction that receives data on position in space, tropism and movements of the body from various sensorial sources (proprioseptive, visual, vestibular) and uses this data to generate a proper motor response to maintain the mass center at the support center.The sensorial, motor and biochemical factors must be considered in evaluation of balance that is a very complicated function including many systems. The investigations must be multi-dimensional to provide data on reasons and grade of the problem. For this purpose, numerous clinical and laboratory methods have been developed from simple to complicated techniques for evaluation of balance function. Balance disorders may be revealed due to many reasons that affect sensorial and motor control systems and may affect the functional capacity, independent life and life quality negatively. An accurate evaluation may ease the treatment-oriented medical rehabilitation and plans for surgical interventions. This review article has dwelled on clinical and evaluation methods that have been used for evaluation of balance function in clinical applications and scientific works.
