Aort cerrahisine ikincil omurilik yaralanmaları

Öz:
Spinal kord (SK) yaralanması; yıkıcı fonksiyonel ve psikolojik etkilerle ilişkili ciddi bir medikal ve sosyoekonomik problemdir. Travmatik olmayan aort acilleri ve bunların tedavileri sırasında meydana gelen spinal kord hasarı vasküler nedenlere bağlı travmatik olmayan SK yaralanmasının bir bölümü olarak değerlendirilir. Aort cerrahisi sonrasında görülen spinal kord hasarı erken veya geç dönemde meydana gelebilir. Erken dönemdeki hasardan medulla spinalisin hipoperfüzyonu ve hipoksisi sorumlu iken; geç dönemdeki hasar ise segmental arterlerin reimplantasyonundaki yetersizliğe veya reperfüzyon hasarına bağlıdır. Cerrahi öncesinde ve sırasında uygulanan çeşitli koruyucu önlemlere rağmen postoperatif spinal kord hasarı halen kayda değer oranlarda göze çarpmakta ve sorun olmaya devam etmektedir. Klinik semptomlar Brown-Sequard sendromundan paraparezi-paraplejiye kadar değişebilir. Semptomlar irreversibl olabilir veya yavaş yavaş düzelebilir. Ayrıca eşlik eden hastalıklar ve medikal komplikasyonlar nedeniyle rehabilitasyon süreci de zorludur.

Spinal cord injury secondary to the aortic surgery

Öz:
Spinal cord injury (SCI) is a serious medical and socioeconomic problem associated to devastating functional and psychological effects. SCI related to nontraumatic aortic pathologies and their treatment was included as a part of the vascular etiology of nontraumatic SCI. SCI after aortic surgery may occur immediately or late. While hypoperfusion and hypoxia of the medulla spinalis is responsible for injury in the early period; the delayed injury depends on the failure of the reimplantation of the segmental arteries or reperfusion injury. In spite of the various protective measures before and during aortic surgery; the postoperative SCI is stil visible at a significant proportion and continues to be a problem. Clinical symptoms range from the Brown-Sequard syndrome to paraplegia and paraparesis. The symptoms may be irreversible or gradually improve. And also the rehabilitation process is challenging because of the concomitant illness and medical complications.
