Ön ve arka üretra darlıklarında üretroplasti: 14 yıllık deneyimimiz

Öz:

Amaç: Bu klinik çalışmada üretra darlığı nedeniyle opere edilen olguların sonuçları sunuldu. Gereç ve yöntem: Şubat 1996–Eylül 2010 tarihleri arasında yaşları 16 ile 63 arasında değişen 55 üretral darlıklı olguya üretroplasti uygulandı. Yirmibir olguda anterior (ön), 34 olguda posterior (arka) üretral darlık vardı. Darlık uzunlukları 1.5 cm ile 14 cm arasında idi. Ön üretra darlıkları, kılsız penil cilt flepleri ve ağız mukozası kullanılarak onarıldı. Arka üretra darlıkları uç uca anastomoz, ağız mukozası, kılsız penil cilt flepleri ile tek tek veya kombinasyon teknikleri kullanılarak onarıldı. Bulgular: Ön üretra onarımlarında bir olguda üretral fistül, bir olguda darlık, bir olguda küçük üretral divertikül gelişti. Arka üretra onarımlarında ise 3 olguda darlık gelişti. Ön üretra onarımlarında fistül gelişen olguda 3 ay sonra fistül spontan olarak kapandı. Darlık gelişen olguda yeniden onarım yapıldı ve onarım sonrası darlık tamamen düzeldi. Divertikül gelişen olgu takibe alındı. Arka üretra onarımı sonrası darlık gelişen 3 olgunun birinde yeniden üretroplasti yapıldı ve sonrası darlık tamamen düzeldi. Bir olguda internal üretrotomi uygulanırken, bir olgu takipten çıktı. Operasyon sonrası ortalama maksimal idrar akımı (Qmax) 24 mL/sn (dağılım 18-35 mL/sn) idi. Hiçbir olguda operasyona bağlı inkontinans gelişmedi. Bir olguda impotans gelişti. Sonuç: Darlık uzunluğu ne olursa olsun, olguya uygun olarak seçilen yöntem ile bütün üretra darlıkları onarılabilir.

Urethroplasty for the anterior and posterior urethral stenosis: our 14 years of experience

Öz:

Objective: In this clinical study, the outcome of subjects operated for urethral stenosis was presented. Materials and methods: Between February 1996 and September 2010, urethroplasty was performed for 55 patients with urethral stenosis aged 16-63 years. Anterior and posterior urethral stenosis was discovered in 21 and 34 cases, respectively. Length of stenosis varied between 1.5 cm and 14 cm. Anterior urethral stenosis was repaired with hairless penil skin fleps and oral mucosa. End to end anastomosis, oral mucosa, hairless penil skin fleps or combinations of these techniques were used for posterior uretral stenosis. Results: The complications of the repair of anterior uretral were urethral fistula in one patient, uretral stenosis in one patient, and uretral diverticula in one patient. Three patients had stenosis after posterior urethral repair. The fistula in the patient operated for anterior uretral stenosis recovered spontaneously after 3 months. The patients with urethral stenosis and diverticula were rerepaired and followed-up without intervention, respectively. Urethroplasty was performed in one of 3 patients who had stenosis following the repair of posterior urethra and complete recovery was obtained, internal urethrotomy was performed in the second patient, and the third patient was out of follow-up. Postoperative mean maximum urine flow rate (Qmax) was 24 mL/sec (range 18-35 mL/sec). None of the patients had incontinence due to operation. One patient complained of impotance. Conclusion: Urethral stenosis can be repaired with appropriate methods regardless of length of stenosis.
