Santral venöz kateter malpozisyonu
Öz: Santral venöz kanülasyon bir çok endikasyonla yaygın şekilde kullanılmakta, bazen hayat kurtarıcı müdahalelere olanak sağlamaktadır. Bununla birlikte, fatal olabilen komplikasyonlara da yol açabilmektedir. En sık karşılaşılan komplikasyon kateterin anormal pozisyonda yerleştirilmesidir. Periferik venöz yolların hastanın gereksimini karşılamakta yetersiz olduğu bir olgu da, sağ subklavian vene santral venöz kateter yerleştirmek amacıyla yapılan müdahalenin sonunda, kontrol PA akciğer grafisinde kateterin sol subklavian vene yerleştiği tespit edildi. İşlem tekrarlanarak kataterin normal pozisyona (vena kava superior) yerleşmesi sağlandı. Anormal pozisyonun yerleştirme esnasında fark edilemeyeceğini akılda tutarak, malpozisyona bağlı komplikasyonlardan kaçınmak için işlem sonunda kontrol grafısi çekilmelidir.

The malposition of the central vein catheterization
Öz: Central vein catheterization is a widely used procedure. At times, it provides possibilities for life saving interventions. However, it may lead to fatal complications as well.The often-encountered complication is the incorrect placement of the catheter. In a case where peripheral veins were insufficient for delivery of the medications, the right subclavian was used to establish a central line. At the end of the application, a control PA/Chest X-Ray revealed that central vein catheter was placed in the left subclavian vein. The procedure was repeated and thecatheter was inserted into its intended correct location ( vena cava superior ). Keeping in mind that the incorrect positioning can occur without any sign, a control PA/Chest X- Ray should be performed in order to avoid further complications.
