1-4 Aylık premature bebeği hastanede tedavi gören annelerin depresyon ve maternal bağlanma ilişkisinin incelenmesi

Öz:

Amaç: Araştırma, hastanede tedavi gören 1-4 aylık prematüre bebeği olan annelerde, doğum sonrası depresyon düzeyi ile maternal bağlanma arasındaki ilişkiyi belirlemek amacı ile tanımlayıcı olarak yapılmıştır. Gereç ve Yöntemler: Araştırmanın evrenini, Ege Üniversitesi Tıp Fakültesi Hastanesi, Dokuz Eylül Üniversitesi Hastanesi, Tepecik Eğitim ve Araştırma Hastanesi ve Dr. Behçet Uz Çocuk Hastalıkları ve Cerrahisi Eğitim Araştırma Hastanesi’nin yenidoğan yoğun bakım ünitesinde (YYBÜ) son bir yıl içerisinde tedavi gören prematüre bebeklerin (N:1623) anneleri oluşturmuştur. Araştırmanın örneklemi, Evreni Bilinen Örneklem Formülü ile belirlenen 140 anneden oluşmuştur. Araştırma verileri, “Anne-Bebek Tanıtım Formu”, “Maternal Bağlanma Ölçeği”, “Edinburg Doğum Sonu Depresyon Ölçeği (ED- DÖ)” kullanılarak toplanmıştır. Bulgular: Annelerin, sosyo-demografik özelliklerine göre dağılımı incelendiğinde; %32.1’inin 27-31 yaş grubunda, %50.7’sinin ilkokul mezunu olduğu belirlenmiştir. Bebeklerin %92.1’inin doğum sonrası yaşlarının 30-60 gün arasında olduğu, yenidoğan yoğun bakım ünitesinde yatış sürelerinin ortalama 38.42±1.60 gün olduğu belirlenmiştir. Annelerin maternal bağlanma puan ortalaması 87.19±5.463, Edinburg Doğum Sonu Depresyon Ölçeği depresyon puan ortalaması 16.000±4.76 olarak belirlenmiştir. Annelerin depresyon puanları ile maternal bağlanma puanları arasında yapılan korelasyon analizinde negatif yönde, zayıf düzeyde, anlamlı bir ilişki olduğu saptanmıştır (r=–0.279, p<0.05). Sonuç: Bu çalışmanın sonucunda yenidoğan yoğun bakım ünitesinde prematüre bebeği olan annelerin maternal bağlanma ve depresyon düzeyleri arasında negatif ilişkinin olduğu saptanmıştır. Bu nedenle, sağlık personeli tarafından aile merkezli bakım yaklaşımının uygulanması, YYBÜ’sinde anne-bebek bağlanmasının gerçekleşmesi ve annelerin depresyon düzeylerin azaltılması için son derece önemli olduğu düşünülmektedir.

Invastigation of correlation between depression and maternal attachment of mothers with 1- to 4-month-old prema- ture babies treated at the hospital

Öz:

Objective: The research was conducted descriptively in order to determine the correlation between postpartum depression and maternal attach- ment of mothers with 1- to 4-month-old premature infants treated at the hospital. Methods: Research environment consisted of mothers of premature infants (N:1623) who were treated in neonatal intensive care units of Ege Univer- sity and Dokuz Eylul University Medical School Hospitals, Tepecik Teaching and Research Hospital, and Behcet Uz Children’s Hospital within the last year. Research sample composed of 140 mothers determined by using a convenience sampling formula regarding the number of premature infants that received treatment at four above-mentioned hospitals. The data were collected using “Mother-Infant Description Form” which identified mothers’ socio-demographic and other characteristics, “Maternal Attachment Scale”, and “Edinburgh Postnatal Depression Scale (EPDS)”. Results: According to the distribution of socio- demographic characteristics, 32.1% of the mothers were in 27-31 years age group, 50.7% were primary school graduates. It was determined that 92.1% of the infants were 30-60 days old, mean length of stay of the infants in neonatal intensive care units was 38.42±1.60 days. Maternal Attachment Scale mean score was 87.185±5.463, EPDS mean score was determined as 16.000±4.745. There was a weak negative significant correlation between maternal attachment scores and depression scores of mothers (r=–0.279, p<0.05). Conclusion: As a conclusion of the study, a negative correlation was determined between maternal attachment and depression of mothers who had premature infants in neonatal intense care unit. Therefore, implementation of family-centered care approach by health-care providers is thought to be extremely important in order to establish mother-infant bonding and reduce levels of maternal depression in NICU.
