Comparison of esmolol to nitroglycerine in controlling hypotension during nasal surgery

Öz:
Amaç: Çalışmamızın amacı nazal cerrahide esmolol ve nitrogliserininkontrollü hipotansiyon oluşturmadaki etkinliklerinin karşılaştırılma- sıdır. Gereç ve Yöntem: Etik kurul onayı alındıktan sonra 40 olgu rando- mize olarak, esmolol (Grup E) veya nitrogliserin (Grup N) şeklinde iki gruba ayrıldı. Grup E’de esmolol 30 sn içinde 500 μg/kg bolus dozverilerek idamede 25-300 μg/kg/dk ve Grup N’de nitrogliserin 0.5-2 μg/kg/dk infüzyon hızında sistolik arter basıncı 80 mmHg olacak şe-kilde uygulandı. Bulgular: Hipotansif dönemde sistolik arter basıncı, diastolik arter basıncı, ortalama arter basıncı ve kalp atım hızı değerlerinde Grup E’de %24, %33, %27 ve %35 (p<0.001, p<0.001, p<0.001, p<0.001), Grup N’de ise %30, %33, %34 ve %23 (p<0.001, p<0.001, p<0.001, p<0.001) azalma izlendi. Hipotansif dönemde kalp atım hızı değerle- rindeki azalma Grup E’de yüksekti (p=0.048). Derlenme döneminde Grup E’de diastolik arter basıncı ve kalp atım hızı değerlerinde %9 ve %18 azalma (p=0.044, p<0.001), Grup N’de ise sistolik arter basıncı, diastolik arter basıncı, ortalama arter basıncı değerlerinde %7, %3 ve %7 (p=0.049, p=0.451, p=0.045) azalma saptandı. Sonuç: Esmololun kontrollü hipotansiyonda benzer hemodinamik değişiklik ve iyi cerrahi görüş alanı sağlaması nedeni ile nitrogliserine alternatif olarak kullanılabileceği kanısındayız.

Nazal cerrahide kontrollü hipotansiyon sağlanmasında esmolol ile nitrogliserinin karşılaştırılması

Öz:
Objective: The aim of this study was to compare esmolol to nitroglycerine in terms of effectiveness in controlling hypotension duringnasal surgery.Materials and Methods: After approval by our institutional EthicsCommittee, 40 patients were recruited and randomized into twodrug groups: esmolol (Group E) and nitroglycerine (Group N). Ingroup E, a bolus dose of 500 &#956;g/kg esmolol was administered over30 sec followed by continuous administration at a dose of 25-300 &#956;g/ kg/min to maintain systolic arterial pressure at 80 mmHg. In group N, nitroglycerine was administered at a dose of 0.5-2 &#956;g/kg/min.Results: During the hypotensive period, systolic arterial pressure, di- astolic arterial pressure, mean arterial pressure, and heart rate were decreased 24%, 33%, 27% and 35%, respectively, in group E (p<0.001, p<0.001, p<0.001, p<0.001) and were decreased 30%, 33%, 34% and 23%, respectively, in group N (p<0.001, p<0.001, p<0.001, p<0.001). The decrease in heart rate was higher in group E during the hypotensive period (p=0.048). During the recovery period, diastolic arterial pressure and heart rate were decreased 9% and 18%, respectively, in group E (p=0.044, p<0.001). Systolic arterial pressure, diastolic arte- rial pressure, and mean arterial pressure were decreased 7%, 3% and 7%, respectively, in group N (p=0.049, p=0.451, p=0.045). Conclusion: Esmolol provides hemodynamic stability and good surgical field visibility and should be considered as an alternative to nitroglycerine.
