A case with schwannoma originating from the sensory sural nerve
Öz: Schwannoma, sinir kılıfının Schwann hücrelerinden kökenalan düzgün sınırlı, kapsüllü ve yavaş büyüyen, nadir görülen iyi huylu bir tümördür. Schwannomalar insan hayatının herhangi bir dekatında ortaya çıkabilmesine rağmen, sıklıkla gençve orta yaş grubunda görülür. Genellikle baş, boyun, medias- ten, retroperitoneal bölge ve ekstremitelerin fleksör yüzlerinde oluştuğu izlenir. Özellikle spinal köklerden, vagus, median, ulnar ve siyatik sinirlerden kaynaklandığı bildirilmektedir. Bu yazıda, bilgimiz dahilinde, sural sinirden köken alan bir Schwannoma olgusu, radyolojik, cerrahi ve histopatolojik bulgulareşliğinde ilk olarak sunulmaktadır. Bacak arka yüzünde şişlik ve çevresinde uyuşma şikayeti ile başvuran 20 yaşındaki erkek olguda magnetik rezonans görüntülerinde gastrokinemius kası içerisinde yerleşmiş, sural sinir trasesi üzerinde bulunan, T2ağırlıklı sagittal kesitlerinde hiperintens, kontrastlı aksiyel ke-sitlerde difüz kontrast tutan ve etrafındaki dokulardan keskin sınırlarla ayrıldığı izlenen kitlesi saptandı. Cerrahisi sırasında sinir kılıfından köken aldığı ve sural sinir fasiküllerini ayrıştırdığı görülen tümörün, total rezeksiyonu yapıldı. Cerrahi sonrası takipte nüks gözlenmedi. Bu şekilde yumuşak doku içerisinde büyüyen büyük boyutlara ulaşabilen tümöral kitlelerin periferik sinirlerden de kaynaklanabileceği akılda tutulmalıdır.

Duyusal sural sinirden kaynaklanan schwannom olgusu
Öz: Schwannoma is a rare, slow growing, encapsulated, benign tumor with regular margins which originates from the schwanncells of the nerve sheath. Although schwannomas may occurat any decade of human life, this tumor is common amongyoung and adult patients. Schwannomas generally developin the head, neck, mediastinum, retroperitoneal region, andover the flexor surfaces of extremities. It is observed to ariseparticularly from spinal roots as well as vagal, median, ulnar,and sciatic nerves. To our knowledge, this is the first reportpresenting a schwannoma case arising from the sural nervealong with the radiologic, surgical, and histopathologic findings. The MR images of a 20-year-old male patient who presented with a swelling at the back of his leg revealed a masswith well-defined borders which was localized along the suralnerve trajectory and within the gastrocnemius muscle, exhibiting diffuse enhancement on the contrast-enhanced axial images and hyperintense appearance on the T2-weighted sagittalimages. During surgery,the tumor, which was observed to arisefrom the nerve sheath and separate the sural nerve fascicles,was subjected to total resection. No recurrence was observedduring the postsurgical period. It should be borne in mind thattumoral developments growing into large tumors within softtissues may also arise from peripheral nerves.
