Akut inme hastalarında hastane içi rehabilitasyon programının etkileri

Öz:
Amaç: Bu çalışma, akut iskemik inme hastalarında hastane içi erken dönemde başlayan rehabilitasyon programının motor, denge, fonksiyonel mobilite ve bağımsızlık işlevleri üzerine etkilerinin belirlenmesi amacıyla yapılmıştır. Gereç-yöntem: Çalışmaya akut iskemik inme tanısıyla yatarak medikal tedavisi yürütülen 42 hasta dahil edilmiştir. Hastane içi rehabilitasyon programının sonuçları Fugl-Meyer Değerlendirme Ölçeği (FMDÖ), İnme Postüral Değerlendirme Ölçeği (İPDÖ), Fonksiyonel Bağımsızlık Ölçeği (FBÖ) ve Fonksiyonel Mobilite Profili (FMP) ile rehabilitasyon programı başlamadan önce ve klinikten taburcu olduğu sırada değerlendirilmiştir. Sosyodemografik ve klinik verilerin sayı ve yüzde dağılımları yapılmıştır. Rehabilitasyon programı öncesi ve sonrasındaki değerlendirme sonuçları eşleştirilmiş t- testi ile, rehabilitasyondan faydalanma oranları Mann-Whitney U testi ile değerlendirilmiştir. Bulgular: Çalışmaya dahil edilen 42 hastanın tüm değerlendirme sonuçları, rehabilitasyon programı sonrasında anlamlı iyileşme göstermiştir (p<0,05). FBÖ sonuçlarına göre yüksek özürlük grubunda yer alan hasta sayısı azalırken tedaviden sonra düşük özürlülük grubundaki hasta sayısı artmıştır. Özürlülük gruplarındaki dağılım açısından hastalar arasında anlamlı fark olduğu tespit edilmiştir (p= 0,000). Kadın ve erkek hastaların tedavi sonrasında tüm değerlendirme parametrelerinde anlamlı iyileşme görülmüş (p<0,05) fakat gruplar arasında anlamlı fark olmadığı tespit edilmiştir (p>0,05). Etkilenen anterior ve posterior serebral dolaşım sistemine göre her iki grupta anlamlı iyileşme görülmüştür (p<0,05). Yaş değişkeni açısından, 55 yaş üzeri grubundaki hastalar, tedavi sonrasında anlamlı iyileşme göstermiştir(p<0,05). Sonuç: Çalışmanın sonuçları, akut inme hastalarında hastane içi erken dönemde uygulanan rehabilitasyon programının motor ve fonksiyonel kapasiteyi iyileştirdiğini göstermiştir. Erkek hastaların ve posterior dolaşım sistemi etkilenmiş hastaların fonksiyonel bağımsızlık yeteneklerinin daha fazla iyileştiği saptanmıştır.

The effects of ınpatient rehabilitation program in acute stroke patients

Öz:
Objective: To determine the effects of inpatient rehabilitation program on motor, balance, functional mobility and independence in acute ischaemic patients. Material Methods: The study was consisted of 42 patients with acute ischaemic stroke during inpatient medical treatment. The outcomes were evaluated with Fugl-Meyer Assessment Scale (FMAS), Postural Assessment Stroke Scale (PASS), Functional Independence Measure (FIM), Functional Mobility Profile (FMP) before rehabilitation program and at the time of discharge from the clinic. Socio-demographic and clinical data were analyzed with the number and percentage distributions. The outcomes were evaluated with Paired t-test and the utilization rate of rehabilitation with Mann-Whitney U test. Results: The outcomes of 42 patients in all scales were improved significantly after rehabilitation program (p<0,05). When the outcomes of FIM were grouped related to disability levels; the number of patients were decreased in high disability group and increased in the lower disability group. There was significant difference between patients in terms of distribution in groups (p= 0,000). All outcomes were improved significantly in female and male patients (p<0,05) but there was no significant difference between the groups (p>0,05). According to the affected cerebral circulation system (anterior-posterior), significant improvements were found in both groups (p<0,05). Patients, older than 55 years old, showed significant improvements after rehabilitation (p<0,05). Conclusion: The results showed that the inpatient rehabilitation program improved motor and functional capacity in acute stroke patients. The functional independence abilities of male patients and patients' affected the posterior circulation system improved furthermore.
