Major karaciğer rezeksiyonunda ERAS protokolü

Öz:
Amaç: Cerrahi uygulamalar ve anestezi tekniklerindeki bü- yük ilerlemeler mortalitede azalmaya neden olmasına rağ- men, elektif şartlarda ameliyata alınan hastalarda uzamış hastanede kalış süresi ve postoperatif morbidite hâlâ sorun olarak görülmektedir. Bu durum klinik bakım standartla- rının ve postoperatif stratejilerin gözden geçirilmesine neden olmuş ve cerrahi kliniklerinde perioperatif bakımın iyileştirilmesine bir yaklaşım olarak cerrahiden sonra hız- landırılmış toparlanma (Enhanced Recovery After Surgery, ERAS) protokolü ya da diğer adı ile hızlandırılmış cerrahi (Fast Track Surgery, FTS) programları geliştirilerek majör cerrahilerde de başarılı bir şekilde uygulanabileceği göste- rilmiştir. ERAS protokolü uygulanan hastalarda organ dis- fonksiyonunun ve morbiditenin azaldığı, hastaneden erken taburcu oldukları daha önce yapılan çalışmalarla gösteril- miştir. Biz de majör karaciğer rezeksiyonu yapılan hasta- larda ERAS protokolü uygulaması hakkında yurt dışında az sayıda, Türkiye’de ise hiç çalışma bulunmadığından, majör karaciğer rezeksiyonu yapılan hastalarda ERAS protokolü uygulaması ile hastaların hastanede kalış süresinin kısaldı- ğını ortaya koymayı amaçladık. Gereç ve Yöntemler: Çalışmamızda majör karaciğer re- zeksiyonu planlanan 40 hastanın, 20’sine klasik bakım yöntemleri (kontrol grubu), 20’sine ise ERAS protokolü uygulayarak hastanede kalış sürelerini karşılaştırdık. Ay- rıca, idrar sondası çekme zamanları, sulu ve katı gıdaya başlama zamanları, ameliyat sonrası erken mobilizasyon zamanı, ameliyat öncesi hareketliliğe ulaşma zamanları da karşılaştırıldı. Bulgular: Kontrol ve ERAS gruplarındaki hastalar hasta- nede kalış süreleri açısından karşılaştırıldığında, istatistik- sel olarak anlamlı fark olduğu saptandı. Kontrol ve ERAS gruplarındaki hastaların ortalama hastanede kalış süresi sırasıyla 11.4±2.84 gün ve 5.5±1.4 gün olarak hesaplandı (p<0.001). Sonuç: Majör karaciğer rezeksiyonu ile beraber ERAS protokolü uygulanan hastaların hastanede kalış süreleri- nin kısaldığı görülmüştür. Çalışmamızın daha önce ERAS protokolü uygulanan çalışmaların sonuçları ile örtüştüğü görülmektedir.

ERAS protocol in major liver resection

Öz:
Objective: Although advances in surgical applications and anesthesia techniques have led to a reduction in mortality, hospitalization time and postoperative morbidity are still troublesome in patients undergoing major liver resecti- on. Therefore, clinical care standards and postoperative strategies were revised. Enhanced Recovery After Surgery (ERAS) protocol or so called Fast Track Surgery (FTS) programs have been evolved and shown that they could be applied successfully for the improvement of perioperative care. It has been shown that organ dysfunction and morbi- dity and hospitalization time were reduced in patients who were applied ERAS protocol. Since there are few researches made on this subject in foreign countries and no research in our country, we conducted this study to show that hospi- talization time is reduced in patients who underwent major liver resection together with ERAS protocol. Material and Methods: Forty patients included in this study, were divided in two groups: Control group (n=20) and ERAS protocol group (n=20). Two groups were com- pared regarding the hospitalization time. Besides, urinary catheter removal time, postoperative time to liquids and so- lid diet, mobilization, and time to return to daily activities were also assessed. Results: Control group and ERAS group were found to be significantly different in terms of hospitalization time. The mean hospitalization time was 11.4±2.84 days in the cont- rol group and 5.5±1.4 days in the ERAS group (p<0.001). Conclusions: Similar to the studies which applied ERAS protocol in the literature, it was shown that hospitalization time was reduced when ERAS protocol was used together with major liver resection.
