Patent ductus arteriosus is a congenital anomaly frequently observed in children, but uncommon in adults. It can be found by systole- diastolic murmur incidentally heard on physical examination or by a shunt flow discovered on echocardiography performed for other purposes in adult patients. The clinical spectrum of patients with patent ductus arteriosus is wide. Patent ductus arteriosus may lead to heart failure and infective endocarditis. Patients without audible murmur and clinical manifestation should be evaluated as silent patent ductus arteriosus. The amount of patients with patent ductus arteriosus increased with the widespread use of Doppler echocardiography examinations. In the therapeutic management of these patient, signs of left ventricular volume overload, the presence of significant shunt and in- creased pulmonary artery pressure must be taken into cosideration. Although routine closure of PDA is not recommended in silent duct, therapeutic management of these patients is controversial.
