Yetişkin hastada intestinal malrotasyon

Öz:

İntestinal malrotasyonlar embriyojenik dönemde ortabarsağın fizyolojik herniasyonu sırasında oluşurlar. Malrotasyonların farklı formlarından dolayı tanısı oldukçazordur. Malrotasyonlar sıklıkla konjenital peritoneal bantlarla ilişkilidirler. Bu bantların en yaygını Ladds bantlarıdır. Bunlar çekumdan subhepatic alana kadar uzanabilirler ve duodenuma baskı yapabilirler. Bunlar infantlarda daha sıktırlar, ancak yaşamın herhangi bir döneminde görülebilirler. İnfantlarda genellikle safralı kusmaşeklinde klinik görünüm mevcutken, yetişkinlerde budurum oldukça değişkendir. Tedavide altın standart Laddsprosedürüdür. Biz 4 yıldır nonspefisik gastrointestinalsemptomları olan, tomografide malrotasyondan şüphelenilen ve Ladds prosedürü ile tedavi edilen 23 yaşındakibayan hastayı sunduk.

Intestinal malrotation in an adult patient)

Öz:

Intestinal malrotations occur during the physiologicalherniation of midgut in the embryonic period. Diagnosisis difficult because of the different forms of malrotations.They are often associated with congenital peritonealbands. The most common of these bands is Ladd s band.They can lie down from the caecum to the sub-hepaticarea, and cause compression of the duodenum. Theseare more frequent in infants yet can occur at any age.Clinical appearance in the adults are quite variable whileit usually occurs in infancy as bilious emesis. In adults,the most common symptom is recurrent bouts of colickyabdominal pain. The gold standart treatment is Ladd sprocedure. We report a case of 23-year-old femalepatient with a 4-year history of non-specific gastrointestinal symptoms and suspected malrotation on thetomography. She was treated by Ladd s procedure.
