Autoimmune pancreatitis is the result of systemicfibroinflammatory disease which mimics pancreaticcancer with the signs of obstructive jaundice, andpancreas enlargement. As it has similarities withpancreas cancer; the symptoms of autoimmunepancreatitis show geographic changes, there have beenseveral diagnostic criteria. Pancreas cancer wassuspected in 83 years old man who had symptoms likeweight loss, jaundice, loss of appetite for 4 months. Hehas been searched by laboratory, radiological andnuclear medicine investigations. Autoimmune pancrea-titis was not suspected according to different diagnosticcriteria. After whipple operation, the patologicalevaluation of the surgical specimen showed autoimmunepancreatitis; also cytomegalovirus inclusion bodies wereseen in the pancreas tissue. Although there have beenseveral diagnostic criteria for autoimmune pancreatitis,they are still inadequate. Our case was out of thediagnostic criteria. Although the etiology of autoimmunepancreatitis has not been found yet, cytomegalovirusshould be thought and investigated in the etiology.
