Bir üniversite hastanesinde bakteriyel ve viral menenjit etkenlerinin farklı PCR yöntemleri ile araştırılması
Öz: Amaç: Menenjit, tanısının hızla konarak erken tedavi yapılmasıgereken komplikasyonları yüksek bir enfeksiyon hastalığıdır.Menenjitlerin çoğunda etiyoloji bilinmeden ampirik tedaviyebaşlanması gerekmektedir. Bu ampirik antimikrobiyal tedaviseçimi, coğrafi bölgedeki yaygın olan patojenlere, hastanın yaşınave o bölgedeki pa tojenlerin antibiyotik duyarlılıklarına göreyapılmaktadır. Çalışmada, menenjit ön tanısı ile takip edilenhastaların beyin omurilik sıvısı (BOS) örneklerinde molekülertestler kullanılarak bakteriyel ve viral etkenlerin belirlenmesiamaçlanmıştır.Yöntem: Çalışmamızda, menenjit ön tanısı ile çeşitli kliniklerdengönderilen farklı hastalara ait 100 BOS örneği incelenmiştir.Bakteriyel menenjit etkenlerinden Streptococcus pneumoniae ,Neisseria meningitidis ve Haemophilus influenzae ; NHS MeningitisReal - TM (Sacace, Italy) ve Speed -Oligo Bacterial Meningitis Test(Vircell, Spain) yöntemleri ile, aseptik menejit etkenlerinden HSVise; HSV I/II Typing Real - TM (Sacace, Italy) ve Attomol HSV 1,2differentiation - DNA- LINA (Vircell, Spain) yöntemleri ilearaştırılmıştır.Bulgular: NHS Meningitis Real - TM yöntemi ile BOS örneklerinde10 S. pneumoniae , 2 N. meningitidis ve bir H. influenzae tespitedilmiştir. Speed -Oligo Bacterial Meningitis yöntemi kullanılarak;4 örnekte S. pneumoniae ve 1 örnekte ise N. meningitidissa ptanmıştır. Bu yöntem ile H. influenzaeya rastlanmamıştır. HSVI/II Typing Real -TM ile 5 BOS örneğinde HSV pozitifliğibelirlenmiştir. Tespit edilen HSVün 3ü HSV tip 2 bulunmuş, 2siise HSV tip 1 olarak saptanmıştır. Attomol HSV 1 -2 yöntemikullanılara k da HSV pozitifliği 3 örnekte belirlenmiştir. Bu yöntemile belirlenen tüm HSVler, HSV tip 1 olarak saptanmıştır.Sonuç: Menenjit hastalarında uygun ve yeterli antimikrobiyaltedavi için öncelikle etkenin belirlenmesi gerekmektedir. PCR gibimoleküler yöntemlerle etkenlerin erken saptanması mümkünolmaktadır. Ortamda bulunan inhibitörler, kontaminant maddelerve DNA izolasyonundan kaynaklanan hatalar ile yönteminstandardize olmaması gibi nedenlerden kaynaklanan yanlış pozitifve yanlış negatif sonuçlara rağmen moleküler yöntemlerinönemini koruduğu düşünülmektedir.

Investigation of bacterial and viral meningitis agents with different PCR methods at a university hospital
Öz: Objective : Meningitis is an infectious disease with high risk ofcomplications that must be diagnosed and treated immediately. Inmost of the cases, meningitis treated empirically without knowingthe etiology. This empirical choice of antimicrobial treatment ismade according to the common pathogens present in thatparticular geographic region and antibiotic susceptibility of thesepathogens and age of the patient. In this study, the aim wa s todetermine bacterial and viral meningitis agents that might bepresent in the CSF of patients who are followed with earlydiagnosis of meningitis.Method : In our study, 100 CSF samples, which belongs to differentpatients, sent from multiple clinics wi th early diagnosis ofmeningitis were analyzed. Among the bacterial meningitis agents,the presence of Streptococcus pneumoniae , Neisseria meningitidisand Haemophilus influenzae were investigated by NHS MeningitisReal - TM (Sacace, Italy) and Speed - Oligo Bacterial Meningitis Test(Vircell, Spain); the presence of the HSV, as one of the agents ofthe aseptic meningitis, was investigated by HSV I/II Typing Real -TM(Sacace, Italy) and Attomol HSV 1 - 2 differentiation - DNA- LINA(Vircell, Spain).Results: With NHS Meningitis Real -TM method, 10 S. pneumonia,two N. meningitidis and one H. influenzae identified from CSFsamples. By using the Speed - Oligo Bacterial Meningitis method , S.pneumoniae was identified in four of the samples and only in onesample N. meningitidis was identified. No H. influenzae wasidentified in any of the samples with this method. With HSV I/IITyping Real -TM, HSV positivity was identified in 5 of the CSFsamples and 3 of them were identified as type 2 and other 2 wereidentified as type 1 . With the use of Attomol HSV 1 - 2 method,HSV positivity was identified in 3 samples. All the HSVs determinedwith the use of this method were identified as type 1.Conclusion : For appropriate and adequate treatment of patientswith meningitis, first of all the agent must be determined. Withmethods like PCR, it is possible to identify the agents early in theinfection. Despite the wrong positive and negative results arisingfrom the reasons like inhibitors present in the environment,contaminant material s, mistakes arising from DNA isolation stepsand being unable to standardize these methods, molecularmethods are thought to maintain their importance.
