Evaluation of bcl-2, bax and c-erbB-2 Levels in Chronic Otitis Patients with or without Cholesteatoma

0, no staining; + cell staining (weak positive staining:Amaç: Bu çalışmanın amacı primer ve sekonder akkizkolesteatomlardaki bcl-2, bax ve c-erbB-2 ekspresyon-larını değerlendirmek ve kolesteatom patogenezindeki hücre proliferasyonu ve apapitozisin rolünü belirle-mektir.Yöntemler: Kolesteatomsuz kronik otitis media (KOM)nedeniyle opere edilen hastaların dış kulak yolu cildin-den (DKY) alınan örnekler grup 1; kolesteatomlu KOMnedeniyle opere edilen grup 3 ve grup 4deki hastalarınDKY cildinden alınan örnekler grup 2; primer akkiz ko-lesteatomlu KOM nedeniyle opere edilen hastaların ko-lesteatom matriksinden alınan örnekler grup 3; sekonder akkiz kolesteatomlu KOM nedeniyle opere edilen has-taların kolesteatom matriksinden alınan örnekler grup4ü oluşturdu. Pozitif hücrelerin oranının değerlendir-mesi belirtilen bulguların varlığı bazında yapıldı ve se-mikantitatif olarak dört grupta sınıflandı: 0, boyama yok;+ hücre boyanması (hafif derecede boyanma: %l-33); ++hücre boyanması (orta derecede boyanma: %34-66); +++1% 33%); ++ cell staining (moderately positive staining:34% 66%); and +++ cell staining (strong positive stain-ing: 67% 100%).Results: Comparison of the staining scores of bcl-2, bax, and c-erbB-2 revealed a statistically insignificant differ-ence in the staining of samples obtained from the EECskin (p>0.05). Decreased bcl-2 expression and increasedbax and c-erbB-2 expressions were determined in pri-mary and secondary acquired cholesteatoma epitheliumcompared with the EEC skin of patients operated forCOM with or without cholesteatoma, and the differ-ences were found to be statistically significant (p<0.05).Conclusion: In acquired cholesteatoma epithelium, thefinding of decreased bcl-2 expression as well as increasedbax and c-erbB-2 expressions compared with the EECskin is an indicator of the increase in both cellular pro-liferation and apoptosis.

Öz:
Objective: The aim of this study was to evaluate bcl-2,bax, and c-erbB-2 expressions in primary and secondaryacquired cholesteatoma and to indicate the role of apop-tosis and accompanying increased cellular proliferation in the pathogenesis of cholesteatoma.Methods: Samples obtained from the skin of the ex-ternal ear canal (EEC) of patients operated for chronicotitis media (COM) without cholesteatoma constitut-ed Group 1; samples from the EEC skin of patients inGroup 3 operated for COM with cholesteatoma andfrom the EEC skin of patients in Group 4 constitut-ed Group 2; samples obtained from the cholesteatomamatrix of patients operated for COM with primary ac- quired cholesteatoma constituted Group 3; and samplesobtained from the cholesteatoma matrix of patients op-erated for COM with secondary acquired cholesteatomaconstituted Group 4.
