Gabapentin vs. pregabalin for the treatment of neuropathic pain in patients with spinal cord injury: A crossover study

Öz:
Amaç: Nöropatik ağrı spinal kord yaralanmasının sık görülen birkomplikasyonudur. Spinal kord yaralanmalı olguların yaklaşık olarak%70inde hastalıklarının bir döneminde ağrı olmaktadır ve bu olgularınüçte biri ağrının şiddetini ciddi olarak tanımlamaktadır. Spinal kordyaralanması sonrasındaki nöropatik ağrının tedavisinde, gabapentin vepregabalin ilk seçenek ilaçlardır. Ancak, literatürde spinal kord yaralanmalıolgularda nöropatik ağrı tedavisinde pregabalin ile gabapentininetkilerini karşılaştıran çalışma yoktur. Biz bu prospektif, randomize,çapraz klinik çalışmada; spinal kord yaralanması (SKY) ile ilişkili nöropatikağrı tedavisinde gabapentin ve pregabalinin etkinliğini karşılaştırmayıamaçladık.Gereç ve Yöntemler: Çalışmaya nöropatik ağrısı olan spinal kordyaralanmalı 30 hasta dahil edilmiştir. Hastalar ilaçlarını 8 hafta boyuncakullanmışlardır. İki haftalık arındırma döneminden sonra, ilaç gruplarıçaprazlama yöntemi ile değiştirilmiştir. Çalışmada hastalar görsel analogağrı skalası (VAS), uyku VASı, Kısa Form- Beck Depresyon Envanteri veağrı dizabilite indeksi üzerinden değerlendirilmiştir.Bulgular: Çalışmayı 21 hasta tamamlamıştır. İlk dönemde kullandığıilacı değiştirmek istemeyen 7 hasta (4ü gabapentin, 3ü ise pregabalinkullanan hasta) ve baş dönmesi ve bulantı gelişen 2 hasta (2si depregabalin kullanan hasta) çalışma dışı bırakılmıştır. Ağrıyı azaltma,anksiyete ve uyku açısından iki ilaç arasında fark saptanmamıştır (p<0,05).Sonuç: Spinal kord yaralanmasına bağlı nöropatik ağrı tedavisinde,çalışma parametreleri açısından gabapentin ve pregabalin arasındaistatistiksel olarak anlamlı fark saptanmamıştır.

Öz:
Objective: Neuropathic pain is a frequent complication of spinal cordinjury. Almost 70% of patients with spinal cord injury (SCI) experiencepain, and nearly one-third of these patients describe their pain as severe.Gabapentin and pregabalin are considered to be frst-line treatmentfor post-SCI neuropathic pain. However, there is no study comparingthe effects of gabapentin and pregabalin in the management ofneuropathic pain in patients with SCI. In this prospective, randomized,crossover clinical trial, we aimed to compare the effcacy of gabapentinand pregabalin in the treatment of neuropathic pain associated withSCI.Material and Methods: Thirty patients with spinal cord injuryexperiencing neuropathic pain were recruited for the study. Patients tookmedications for 8 weeks. After a 2-week washout period, medicationswere switched in a crossover design. The outcome measures for thisstudy were visual analogue scale (VAS) for pain, VAS for sleep, ShortForm Beck Depression Inventory, and the pain disability index.Results: Twenty-one patients completed the study. Seven patients whodid not want to change their medication while they were using thefrst drug (4 patients were using gabapentin and 3 patients were usingpregabalin) and 2 patients who experienced dizziness and nausea (bothwere in the pregabalin group) were dropped off. There was no differencebetween both drugs in terms of their effcacy for pain, anxiety, and sleep(p<0.05).Conclusion: There was no statistically signifcant difference betweengabapentin and pregabalin in terms of study parameters for thetreatment of neuropathic pain in patients with SCI.
