Reliability of different predictive systems in major gastrointestinal surgical patients

Öz:
APACHE II- III, SAPS II ve MPM II sık kullanılan prediktif modellerdir. ASA ve P -POSSUM cerrahi hastalar içindizayn edilmişlerdir. NRS - 2002 skorlaması nütrisyonel riskin taranması için önerilmektedir. Bu çalışmada,prognostik modellerin etkinliğini değerlendirmek ve major ga strointestinal cerrahi hastalarda bu modeller ileNRS 2002'nin güvenilirliğini karşılaştırmak amaçlanmıştır. Major gastrointestinal cerrahi uygulanan hastalarınbaşvuru anındaki APACHE II ve III, SAPS II, MPM II ve P -POSSUM skorları, ASA ve NRS - 2002 skorla rı kayıtedildi. Ameliyat sonrası 24.saatte APACHE II ve III, SAPS II,ve MPM II skorları kayıt edildi. Skorlamasistemlerinin farklılığı ve kalibrasyon özellikleri değerlendirildi. Postoperatif 24. saatte APACHE II - III, SAPS IIve hastaneye başvuruda P -POSSUM'un major gastrointestinal cerrahiye giden hastaların mortalitetahminindeki ayırt edici ve değerlendirici gücü güvenilirdi. APACHE III, SAPS II ve P - POSSUM komplikasyontahmini için en iyi performansı gösteriyordu. ASA, MPM II, ve NRS -2002' nin ise bu hastaları ayırt edici gücüyetersizdi. APACHE II ve III, SAPS II ve P - POSSUM gastrointestinal cerrahi hastalarda mortalite tahminindediğer hastalara göre üstündür. Komplikasyonların tahmininde ise APACHE III, SAPS II ve P - POSSUM üstündür.

Major gastrointestinal cerrahi hastalarında farklı prediktif sistemlerin güvenilirliği

Öz:
APACHE II- III, SAPS II, and MPM II are commonly used predictive models. The systems designed for surgicalpatients include the ASA and P - POSSUM. NRS - 2002 score is suggested for screening of nutritional risk. Weaimed to assess the pe rformance of prognostic models, and to compare the reliability of NRS - 2002 with those,in major gastrointestinal surgical patients. APACHE II and III, SAPS II, MPM II and P -POSSUM scores, ASAgrading, and NRS - 2002 scores of the patients underwent major gas trointestinal surgery were collected onadmission. Calculations were repeated for APACHE II and III, SAPS II, and MPM II at postoperative 24 th hour.Discrimination and calibration characteristics of the scoring systems were evaluated.APACHE II - III, and SAPS IIat postoperative 24 th hour, and P - POSSUM on admission, had reliable power of discrimination and calibrationfor mortality prediction in patients undergoing major gastrointestinal surgery. APACHE III, SAPS II and P - POSSUM had the best performance on complication prediction. ASA, MPM II, and NRS - 2002 had insufficientdiscrimination statistics. APACHE II and III, SAPS II, and P - POSSUM are superior to other systems in predictingmortality in gastrointestinal surgical patients. APACHE III, SAPS II, and P -P OSSUM are superior in predictingcomplication.
