Functional Outcomes to Surgical Treatment of the Distal Humerus Intra-Articular Fractures in Adults

Öz:

Amaç: Humerus alt ucunu ilgilendiren eklem içi ve yer değiştirmiş kırıklar cer¬rahi tedavi gerektirmektedir. Bu çalışmamızda humerus alt ucunuilgilendi¬ren eklem içi kırıklara uyguladığımız cerrahi tedavinin fonksiyonelsonuçları¬nı ve sonuçlara etki eden faktörler değerlendirildi. Gereç ve Yöntem: 2005 ve 2010 yılları arasında kliniğimizde tedavi olan humerus alt ucueklemi ilgilen¬diren erişkin 21 hastanın 22 kırığı retrospektif olarak incelendi. Hastalarımı¬zın 10 (%47,6)u bayan, 11 (%52,4)i erkek olup ortalama yaş46,2 (17-71) idi. AO sınıflamasına göre kırıkların 10 (%46)u tip B-3, 6 (%27)tip C-1, 6 (%27) tip C-2 olarak sınıflandırıldı. Kırık tespiti amacıyla kırıkların 9(%41)unda pa¬ralel kilitli çift plak, 2 (%9)inde dik açılı çift plak, 7 (%32)indeK teli ve vida kombinasyonu, 4 (%18)ünde tek plak uygulandı. Sonuçlar MAYOve q-DASH skorlamalarına göre değerlendirildi. Bulgular: Hastalarımızın ortalama izlem süresi 30,4 (5-68) ay idi. Kırıkların hepsi kaynama görüldü. Ortalama dirsek fleksiyonu 117 (90-145) derece, ekstansiyon kaybı 24,3 (0-60)derece olarak saptandı. Mayo dirsek performans skorlamasına göre 10 (%47)hastada mü¬kemmel, 6 (%29) hastada iyi, 3 (%14) hastada orta, 2 (%10)hastada kötü so¬nuç elde edildi. 21 hastanın q-DASH skorlaması ortalama15(0-69,8) idi. İyi sonuçların kırık tipine göre stabil osteosentez yapılan, erken cerrahi uygula-nan ve erken rehabilitasyona başlanan hastalarda eldeedildiği tespit edildi. Tartışma: Humerus alt ucu ilgilendiren kırıkların tedavisinde; anatomik redüksiyon ve stabil osteosentezin erken rehabilitasyona izinvermesi, başarılı sonuç alınmasında oldukça önemli olduğu; ayrıca kırık tipi vehastanın yaşınında sonuçlar üzerinde etkili olduğu sonucuna varıldı.

Erişkin Distal Humerus Eklem İçi Kırıklarının Cerrahi Tedavisinin Fonksiyonel Sonuçları

Öz:

Aim: Displaced and intraarticular humerus distal fractures require surgicaltreatment. In this study we evaluated to functional results of the surgicaltreatment of distal humerus intraarticular fractures in adults. Additionally,we evaluated the factors affecting surgical outcomes. Material and Method:Between 2005 to 2010, 22 elbows of 21 patients who were operated fordistal humerus intraarticular fractures were evaluated retrospectively.11(52.4%) patiens were male and 10(47.6%) were female, mean age 46.2(range 17 to 71). Fractures were classified as type B3 for 10, type C1 for6 and type C2 for 6 subjects according to AO classifications. We performedparalel double locking plate for 9(41%), perpendicular double plate for 2(9%),K wires and screw combination for 7(32%), and single plate for 4(18%)patients. Functional assesment were performed with the MEPS and q-DASHscoring systems. Results: Mean follow-up was 30.4(5-68) months. Fracturehealing was observed in all of the patients. Mean elbow flexion of 117 (90- 145) degrees and extension loss of 24.3 (0-60) were noted. 10 (47.6%)patients were excellent, 6 (28.6%) were good, 3 (14.3%) were fair and 2(9.5%)were poor according to MEPS. Mean q-DASH scores were 15 (0-69.8) in 21patients. Good results were obtained in fractures treated with stable fixation,in early performed surgeries, and in early started rehabilitation. Discussion:Treatment of distal humerus intraarticular fractures with anatomicalreduction, stable osteosynthesis, and early rehabilitation are very importantin obtaining successful outcomes. Additionally, age and fracture type areimportant factors to conclude good results..
