Psychiatric Symptoms in Male Traumatic Lower Limb Amputees: Associations with Neuropathic Pain, Locomotor Capabilities, and Perception of Body Image

Öz:
Amaç: Bu çalışmanın amacı nöropatik ağrısı olan ve olmayan alt ekstremite amputasyonlu erkek hastaları psikiyatrikbelirtiler açısından karşılaştırmak ve bu belirtilerin işlevselkapasite ve beden algısı ile ilişkisini belirlemektir. Yöntem: Çalışmaya 50 travmatik alt ekstremite amputasyonlu erkek hasta alınmıştır. Bunların 27sinde nöropatikağrı saptanırken (Grup 1), diğer 23ünde saptanmamıştır(Grup 2). Hastaların yaş, kilo, boy ve amputasyondansonra geçen süre gibi demografik bilgileri, kaydedilmiştir.Çalışmaya katılan her bir hastanın amputeler için bedenalgısı skoru ve lokomotor kapasite indeksi skoru hesaplanmıştır. Tüm hastalar ayrıca Belirti tarama testi-90R,Beck Depresyon Envanteri, Pittsburg Uyku Kalitesi Ölçeği,Rosenberg Benlik Saygısı Ölçeği, Durumluk-Sürekli KaygıEnvanteri ve Kısa Form-36 yaşam kalitesi ölçeğini tamamlamıştır.Bulgular: Nöropatik grupta somatizasyon, kişilerarası duyarlılık, depresyon, anksiyete, öfke-kızgınlık, uykubozukluğu ve yaşam kalitesi düşüklüğü belirtileri anlamlıolarak daha yüksek saptanmıştır. Psikiyatrik belirti puanlarıile ampute beden algısı ölçeği ve lokomotor kapasite ölçeğipuanları arasında anlamlı korelasyon saptanmıştır. Sonuç: Nöropatik ağrısı olan ampute hastalarda anksiyeteve depresyon belirtilerinin yanısıra başka psikiyatrik belirtiler da görülebilir. Artmış fiziksel aktivite ve olumlu bedenalgısı psikiyatrik belirtilerde azalma ile ilişkili olabilir.

Öz:
Aims: The aim of this study was to compare psychiatricsymptoms of male lower limb amputation (LLA) patientswith neuropathic pain and without neuropathic pain;to show the relationship between these outcomes andfunctional status and body image disturbances. Methods: Fifty male traumatic LLA patients were includedinto the study. Twenty-seven were diagnosed withneuropathic pain (Group 1), while 23 had no neuropathicpain (Group 2). Demographic data including age, height,weight, and duration of amputation were recorded. Theamputee body image scale (ABIS) score, and locomotorcapabilities index (LCI) score were calculated for eachparticipant. They have also completed the SymptomChecklist-90-R, Beck Depression Inventory, Pittsburgh SleepQuality Index, Rosenberg Self-Esteem Scale, State TraitAnxiety Inventory, and Short Form 36.Results: Intergroup comparison revealed significantdifferences in somatization, interpersonal sensitivity,depression, anxiety, anger-hostility, sleep quality, sleepduration, sleep disturbance, daytime sleepiness, traitanxiety and QOL. The differences were statisticallysignificantly higher in Group 1 when compared to Group 2.There were significant correlations among the ABIS and LCIscores and psychiatric symptom severity.Conclusion: Psychiatric symptoms other than depressionand anxiety may also be present in amputees withneuropathic pain. Increasing physical activity and a positivebody image perception may be associated with diminishedpsychiatric symptoms.
