Fracture of Posterior Vertebral Endplate Mimicking Lumbar Disc Herniation: Case Report

Öz:

Fizik tedavi kliniğine altı aydır süren bel ve sol bacak ağrısı ile başvuran 21 yaşındaki erkekhastayı sunuyoruz. Lomber omurganın manyetik rezonans görüntüleme'sinde (MRG) L5-S1santral disk herniasyonu saptandı ve hastaya 2 hafta süreyle fizik tedavi uygulandı. Ancakhastanın semptomları daha kötü oldu, ve cerrahi tedavi almadan önce ek radyolojik tetkiklerplanlandı. Lomber vertebranın X-ray ve bilgisayarlı tomografi (BT) tetkikinde lomber beşinciveretebrada (L5) son plak kırığı saptandı. Manyetik rezonans görüntüleme (MRG) lomberdisk herniasyonunun teşhisinde standart yöntemdir, ancak lomber disk herniasyonunu takliteden diğer hastalıklar MRG'de gözden kaçabilir. Yanlışlıkla lomber disk herniasyonu olarakteşhis edilebilecek birçok hastalık olduğu için, MRG dışındaki diğer radyolojik görüntülemeyöntemleride uygun cerrahi stratejiye sahip olmak için kullanılmalıdır.

Lomber Disk Herniasyonunu Taklit Eden Posterior Vertebral Son Plak Kırığı: Olgu Sunumu

Öz:

We report a 21-year-old man who was admitted to physical therapy clinic and had low backand left leg pain for 6 months. Magnetic resonance imaging (MRI) showed L5-S1 central discherniation and the patient underwent physical therapy for 2 weeks. However patient'ssymptoms were worsened, and before surgical intervention, more radiologic investigationswere planned. X-ray and computed tomography (CT) scan showed an L5 posterior endplatefracture. MRI is the standard modality for diagnosing lumbar disc herniation, but otherdiseases mimicking lumbar disc herniation may be overlooked in an MRI. As there are manydifferent diseases that might be misdiagnosed as lumbar disc herniation, for accuratediagnosis, other diagnostic modalities besides MRI should be used to provide the appropriatesurgical strategy.
