The Effect of Cholecystectomy on the Histology of Antrum and Helicobacter Pylori Colonization

Öz:

Amaç: Kolesistektomi semptomatik safra taşları için standart tedavi olmasına rağmen, duodenogastrik reflüyü artıran ve bununla ilişkili klinik ve laboratuar bulgulara sebep olan değişikliklere yol açabilir. Bu çalışmanın amacı kolesistektominin duodenogastrik reflü, antral mukozada histopatolojik değişiklikler ve Helikobakter pilori kolonizasyonu üzerine etkisini araştırmaktır. Gereç ve Yöntem: Bu prospektif çalışmaya kolesistektomi ameliyatı yapılacakolan 45 hasta ( 28 kadın, 17 erkek; yaş ortalaması: 50.9) alındı. Tüm hastalaraameliyat öncesi ve ameliyattan en az iki ay sonra olmak üzere üst gastrointestinal endoskopi yapılarak midede safra varlığı değerlendirildi. Antrumdanalınan endoskopik biyopsilerde mukozadaki histopatolojik değişiklikler (kronik gastrit, aktivasyon bulguları ve intestinal metaplazi) ve Helikobakter pilorivarlığı incelendi. Bulgular: Safra reflüsü, kronik gastrit ve intestinal metaplazi kolesistektomi sonrasında anlamlı olarak yüksek bulundu (safra reflüsü vekronik gastrit için p<0.001, intestinal metaplazi için p<0.05). Aktivasyon bulguları ve Helikobakter pilori varlığı açısından kolesistektomi öncesi ve sonrası anlamlı fark yoktu. Tartışma: Kolesistektominin duodenogastrik reflüye veantrumda histopatolojik değişikliklere yol açmakla beraber H.pylori kolonizasyonunu etkilemediğini düşünmekteyiz.

Öz:

Aim: Notwithstanding cholecystectomy s being the standard cure for symp- tomatic gallbladder stones, it might as well trigger changes that result in theincrease of duodenogastric reflux and the emergence of relevant clinic andlaboratorial data. The aim of this thesis is to explore the effect of cholecys- tectomy on the duodenogastric reflux, histopathologic changes in the antralmucosa and Helicobacter pylori colonization. Material and Method: This pro- spective research is based upon data collected from 45 patients (28 of thembeing female and 17 of them being male patients) with an average age of50.9, who would undergo cholecystectomic surgery. Upper gastrointestinalendoscopy has been performed on patients before the operation and at leasttwo months after the operation in order to examine the existence of bile inthe stomach. The tissues removed from the antrum during these endoscopicbiopsies have been examined to spot the histopathologic changes and theexistence of Helicobacter pylori in the mucosa (chronic gastritis, activationfindings, and intestinal metaplasia). Results: Duodenogastric reflux, chronicgastritis, and intestinal metaplasia have been observed to increase significantly after cholecystectomy (p<0.001 for duodenogastric reflux and chronicgastritis, p<0.05 for intestinal metaplasia). On the other hand, no significantdata have been attained in terms of activation findings and the existence ofH.pylori before and after the operation. Discussion: Although cholecystectomy cause duodenogastric reflux and histopathologic changes in the antrum,it does not affect H.pylori colonization.
