Effect of Visuospatial Neglect on Electrophysiological Parameters of Oropharyngeal Swallowing
Amaç: Görsel uzaysal ihmalin orofaringeal yutmanın elektrofizyolojik parametrelerine etkisini değerlendirmek.Gereç ve Yöntemler: Çalışmaya 26 sağlıklı gönüllü ve yutma şikayeti olan 42 inmeli hasta dahil edildi. Hastalar görsel uzaysal ihmali olan sol hemiplejik hastalar, görsel uzaysal ihmali olmayan sol hemiplejik hastalar ve görsel uzaysal ihmali olmayan sağ hemiplejik hastalar olmak üzere 3 gruba ayrıldı. Hastalar su içerken, submental elektromyografik aktivite ve laringeal vertikal hareketler elektrofizyolojik olarak kayıtlandı.Bulgular: Faringeal yutmanın tetiklenmesi için geçen süre (A-0) ve orofaringeal yutmanın faringeal faz süresi (A-C) görsel uzaysal ihmali olan sol hemiplejik hastalarda diğer gruplara göre anlamlı olarak uzamıştı (p<0,05). Görsel uzaysal ihmali olan sol hemiplejik hastalarda diğer gruplarla karşılaştırıldığında 0-2 aralığı uzamış ve disfaji limiti azalmıştı. Ancak bu uzama ve azalma sağlıklı gönüllü grupla karşılaştırılmada anlamlı iken (p<0,05), görsel uzaysal ihmali olmayan hemiplejik hastalarla karşılaştırmada istatistiksel olarak anlamlı değildi (p>0,05). Sonuç: Algısal eksiklikler, orofarinksin uyaran algılama ve tanımlama kapasitesinde azalma yaparak, disfaji gelişiminde önemli rol oynarlar. Sonuç olarak, ihmali olan hastalar, disfaji şikayeti olsun ya da olmasın, yutma problemleri açısından değerlendirilmelidirler
