Awareness and Management Delirium in Primary Care

Öz:

Deliryum, birinci basamakta, hastanede yatan yaşlılarda ve bakımevlerinde sık karşılaşılan ciddi bir sağlık problemidir. Tanının geç konması ve dolayısıyla tedavinin başlanmasında gecikmeler olabilmektedir. Bu durum hasta ve bakım sağlayıcıları olumsuz etkilemektedir. Deliryum, hastane yatış süresini uzatmakta, morbidite ve mortalite riskini de arttırmaktadır. Ayrıca, sağlık giderleri üzerine de olumsuz etkileri olmaktadır. Deliryum tanısı, klinik olarak konmaktadır. Klinik bulgular, değişkenlik göstermesi nedeni ile tanısı geç konabilmektedir. Yeterli bir anamnez, fizik muayene ve gerekli laboratuar testleri ile, sıklıkla tanı ve tedavisi mümkün olabilmektedir. Çevresel düzenlemelerin yanı sıra, aile bireylerinin hastanın yönetimine katılımı ve çeşitli medikal tedaviler (nöroleptikler ve benzodiazepinler) kullanılmaktadır. Bu çalışmada, deliryum hakkındaki klinik bilgilerin toplanması ve Aile Hekimliğinde, bu konu hakkındaki farkındalığın arttırılması amaçlanmıştır.

Öz:

Delirium is an important health problem especially for advanced age. The delay of diagnosis causes delays to initiate treatment. This has a negative effect on patients and caregivers. Delirium has a 1-2% prevalence for patients over 65 years. It increases the risk of mortality and morbidity for elder patients. Diagnosis of delirium is made clinically. Clinical findings may show a variety of features which may delay the diagnosis. A careful patient history, physical examination, and accurate laboratory tests may help with diagnosis and treatment. In addition to environmental arrangements, the contribution of family members and drugs (neuroleptics, and benzodiapines) are required. This manuscript aims at increasing awareness and management of delirium in primary care and collection of clinical data on delirium.
