Asemptomatik okul çağı çocuklarında allerjenlerle deri testi pozitifliği

Öz:

AMAÇ: Bu çalışmada atopik hastalık öyküsü olmayan ilkokul 3. ve 5. sınıf öğrencilerinde deri testi pozitifliği incelendi. YÖNTEM: Okullar sosyoekonomik düzeylerine göre yüksek, orta ve düşük olmak üzere üç guruba ayrıldı. Çocuklara ve ailelere bir anket uygulandı. Bu ankette allerjik hastalık semptomları ve ailede atopik bünye öyküsü sorgulandı ve izin alındı. 441 çocuğa multitest aplikatör ile mite, epidermal karışımlar, çayır ve ot polenleri, küf mantarları ve süt ile deri testi uygulandı. 3mm ve üstündeki ödem plağı pozitif olarak değerlendirildi. SONUÇLAR: Herhangi bir allerjene karşı pozitiflik oranı % 16.5 idi. Bu oran 5. sınıflarda %23.3, 3. sınıflarda %15.3 idi. Allerjenlere göre ayrıldığında mitelere karşı % 8.84, epidermal karışımlara karşı % 4.98, çayır polenleri, küf ve süte karşı % 1.36 şar ve ot polenlerine karşı % 0.09 duyarlılık vardı. Pozitif deri testi olanların % 20.48 inde birden fazla allerjene duyarlılık saptandı. Sosyoekonomik düzeylere göre deri testi pozitifliği sırasıyla yüksek, düşük ve orta düzeylerde giderek azalıyordu. Aralarında ki farklar istatistiksel olarak anlamlı idi (p=0.001, p=0.013). YORUM: Genetik ve çevresel faktörlerin birlikte rol oynadığı allerjik hastalıklarda erken önlemler duyarlılaşmanın gecikmesini sağlayabilecek olsa da deri testleri rutin çocukluk tarama testleri arasına girmemelidir. Ailede atopik hastalığı olan asemeptomatik bireylere istek üzerine önerilebilir.

Skin-prick test positivity in asymptomatic school-aged children

Öz:

PURPOSE: In this study, we investigated skin prick test (SPT) positivity in 3th and 5th year primary school children without allergic symptoms. METHODS: These school were divided to three groups according to their socio-economic status as high, middle and low. We gave a questionnaire to the parents and children in order to find out if the children had an allergic diseases and positive atopic family history. And informed constent was obtained from the parents. SPT was performed by multitest aplicator with mite, epidermal mixes, grasspolun, weed, molds and cow's milk. SPT results was considered positive if wheal diameter was >0.3mm. RESULTS: A positive reaction to at least one allergen was observed in %16.5 children. This rate is 23.3 % in 5th students and %15.3 in 3th students. Positive reaction to various allergens were as follow; %8.84 to mite, %4.98 to epidermal mixes, %1.36 to grasspolen, mold and milk, and %0.09 to weed allergens. Multi-allergen sensitivity was %20.8 in children with positive SPT. According to socio-economic status SPT positivity were decreased high, low and middle, respectively. There were statistically difference between groups (p=0.001,p=0.013). CONCLUSION: Skin prick test is not recommended as a screening method in asymptomatic children. SPT could be performed as parents request in asypmtomatic children with family history of atopy.
