Efüzyonlu diz osteoartritinde hyaluronan tedavisinin etkinliği: Bir yıllık plasebo kontrollü çalışma

Öz:

Bu çalışmanın amacı intraartiküler Hyaluronan (HA) tedavisinin efüzyonlu diz osteoartritinde (OA) etkin-liğini araştırmaktır. Bir yıl süreli, tek kör, plasebo kontrollü ve randomize bir çalışma planladık. Çalışmaya primer diz OA ta-nısı konulan 41 hasta alındı, ilaç grubundaki hastalara intraartiküler HA, plasebo grubundaki hastalara da intraartiküler serum fizyolojik birer hafta ara ile 3 defa uygulandı. Klinik etkinlik WOMAC ağrı, tutukluk ve fiziksel fonksiyon skorları, 25m yürüme süresi, eklem hareket açıklığı, efüzyon miktarı ve diz çevresi ölçümü kullanılarak tedavi öncesinde, tedavi tamamlandıktan 1 hafta sonra ve 1 yıl sonra değerlendirildi. Tedavi tamamlandıktan Ihafta sonra ilaç grubunda efüzyon miktarında anlamlı azalma saptanırken (p=0.001) plasebo grubunda fark saptanmadı (p=0.133). Bir yıl sonraki değerlendirmede WOMAC sert-lik skorunda ve diz çevresi ölçümlerinde ilaç grubunda anlamlı azalma izlenirken (p=0.001 ve p=0.0001 sırasıyla) plasebo grubunda fark izlenmedi (p=0.064 ve p=0.349 sırasıyla). Yine 1 yıl sonrasında WOMAC ağrı skoru hariç (p=0.317), WOMAC fiziksel fonksiyon skoru, eklem hareket açıklığı ve yürüme süresi üzerine HA tedavisinin plasebodan anlamlı olarak üstün olduğu saptandı (p<0.05). Enjeksiyonlar sırasın-da ve sonrasındaki takip dönemlerinde hiçbir hastada sistemik veya lokal yan etkiye rastlanmadı. Sonuç olarak çalışmamız efüzyonlu diz osteoartriti olan hastalarda intraartiküler HA enjeksiyonunun kli-nik parametreler üzerine etkinliğinin plasebodan üstün olduğunu, bu etkinliğin 1 yıl devam ettiğini ve gü-venli bir tedavi şekli olduğunu göstermiştir.

The efficacy of hyaluronan treatment on the knee osteoarthritis with effusion: One year placebo conrolled trial

Öz:

The aim of the study was to investigate the efficacy of the intraarticular Hyaluronan (HA) treatment on the knee osteoarthritis (OA) with effusion. The study was one year duration, single blind, placebo controlled and randomized. Forty-one patients with primary knee OA were enrolled in the study. The patients were divided into two groups as drug and placebo groups. The patients in the drug group were administered intraarticular HA and the patients in the placebo group were administered intraarticular 0.9%NaCl, once a week, totally three times. Clinical efficacy was assessed by WOMAC pain, stiffness and physical function scores, 25 m walking time, knee flexion, effusion volume and measurement of knee circumference before the treatment, one week and one year after the treatment. While statistically significant decrease of the effusion was determined in the drug group (p=0.001), the-re was no difference in the placebo group (p=0.133) one week after the treatment. In the drug group in WOMAC stiffness score and measurement of knee circumference, statistically significant decrease we-re determined one year after treatment (p=0.001 and p=0,0001 respectively) but there was no differen-ce in the placebo group (p=0.064 and p=0.349 respectively). In addition, 1 year after the treatment in the drug group in WOMAC physical function score, measurement of knee circumference and 25 m walking time, was found to be significantly better than placebo group except WOMAC pain score (p=0.317), and significant differences were found one year after the treatment (p<0.05). There was no any local or syste-mic reactions after the injections or in the follow-up period.
