Stiff Person Sendromlu Olguda TİVA ile Anestezi Yönetimi ve Literatür Derlemesi
Öz: Stiff Person Sendromu (SPS), nadir görülen, aksiyal ekseni ve ekstremite kaslarını etkileyen, ilerleyici rijidite ve kas spazmları ile karakterize nörolojik bir hastalıktır. Spinal kord aracılı refleks mekanizmanın fizyolojik düzensizliği ile ilişkili ani hareketler görülür. Mekanizması tanımlanmamışsa da beyin omurilik sıvısında GAD (Glutamik asit dekarboksilaz) antikorlarının bulunması ve diğer otoimmün hastalıklarla birlikteliği immünolojik kaynaklı olduğunu düşündürmektedir. Semptomatik tedavide benzodiyazepinler, baklofen ve immünsupresiflerden faydalanılır. Sendromun tedavisinde kullanılan ilaçların anestezik ajanlarla olası etkileşiminden dolayı anestezi uygulamaları özellik gösterir. Bu yazıda; SPS tanılı erişkin olguda bilateral el bilek tendon onarımı operasyonunda, TİVA ile anestezi yönetimimizi sunarak literatür bilgileri ışığında tartışmayı hedefledik

Anesthetic Management of Patient with Stiff Person Syndrome, and Literature Review
Öz: Stiff Person Syndrome (SPS) is a rare neurological disorder characterized by progressive rigidity and muscular spasms affecting the axillary axis and extremity muscles. Spinal cord-mediated reflexes show sudden movements associated with physiological irregularity. The presence of GAD antibodies in the cerebrospinal fluid and coincidence with other autoimmune diseases suggest an immunological basis. Benzodiazepines, baclofen and immunosuppressives are used for symptomatic treatment. Anesthesia is indicated because of the possible interaction of drugs used in the treatment of the syndrome with anesthetic agents. We aimed to discuss in the light of the literature information by presenting our anesthesia management with TIVA in bilateral hand wrist tendon repair operation as adult with SPS
