Elyaf takviyeli polimer kompozit malzemelerin farklı delme yöntemleri ile delinmesinde kesme performansının değerlendirilmesi: Literatür Taraması

Öz:
Elyaf takviyeli polimer kompozit (ETPK) malzemeler, yüksek mukavemet ve sönümleme, hafiflik, düşüktermal genleşme gibi mühendislik özelliklerinden dolayı uzay ve havacılık sanayisinde cıvatalı, perçinli vepimli bağlantılarda yaygınlıkla kullanılmaktadır. Ancak, metallere göre daha karmaşık yapıya sahip olduklarıiçin delme işlemi esnasında delik giriş ve çıkışında delaminasyon, matris fazında bozulma ve elyaf kopmasıgibi etkenler, bu malzemelerin sahip olduğu mühendislik özelliklerinin azalmasına sebep olmaktadır. Bumalzemeler, uzay ve havacılık sektöründe özellikle gövde-kanat bağlantıları için titanyum ve alüminyumgibi metaller ile birlikte istiflenerek delinmektedir. İstiflenmiş malzemelerin herbiri, farklı özelliklere sahipolduğundan, delme işlemi esnasında hassas işlenmiş yüzey kalitesi için optimum kesme şartlarınınbelirlenmesi gerekmektedir. Bu çalışmada, ETPK malzemelerin yekpare ve metaller ile istiflenmiş haldedelinmesi ile ilgili literatürde yapılan çalışmalar araştırılarak elde edilen bulgular bir arada sunulmuştur.

Akromegalili olguda gelişen postoperatif ciddi hipertansiyon

Öz:
Akromegali, hipofiz adenomuna bağlı büyüme hormonuaşırı salınımı ve yükselmiş insülin benzeri büyüme faktörü-1 (IGF-1) ile karakterize olan bir hastalıktır. Bu hastalardahipertansiyon %18-60 olguda görülüp yaşla birlikteartmaktadır ve diyastolik kan basıncında artış ön plandadır.Artan plazma hacmi, ANP düzeylerinde azalma, insülindirenci ve hastalık süresi, hipertansiyon riskini artıran eşlikeden faktörler hipertansiyona neden olur. muhtemeldir.Bu makalede pitüiter mikroadenoma sekonder akromegalibulunan bir kadın olguda postoperatif ciddi hipertansiyontablosu sunulmuştur.

Horlama şikayeti ile başvuran obstruktif uyku apnesi hastalarında üst hava yolu kollabsibilitesinin değerlendirilmesi

Öz:
Amaç: Tıkayıcı uyku apnesi sendromu olan hastalarda başarılı cerrahi için obstrüksiyon alanlarının ve obstrüksiyon boyutunun değerlendirilmesi ve tesbiti için en uygun tanısal yöntemi saptamaktır. Gereç ve Yöntem: Çalışmamıza horlama şikayeti ile başvuran septoplasti operasyonu sonrası polisomnografisi de yapılan 40 hasta dahil edildi. Hastaların yaş, cinsiyet, Epworth uyku skalası, vücut kitle endeksi, modifiye Mallampati skoru, Müller manevrası ve uyku endoskopisi değerlendirildi. Hastaların Müller manevrasında ve uyku endoskopisinde tıkanıklığı %50 üzerinde olanlar ciddi obstrüksiyon olarak değerlendirildi. Bulgular: Laringeal bölgede uyku endoskopisi ile modifiye mallampati skorları arasında anlamlı farklılık tesbit edilmedi. Uyku endoskopisi ve Müller manevrası değerlendirildiğinde iki yöntem arasında uvulopalatal bölgede, lateral faringeal band ve dil kökünde anlamlı farklılık izlendi. Sonuç: Uyku sırasında önemli değişiklikleri gözlemlenmesinden dolayı uyku endoskopisi daha sık olarak tercih edilmektedir. Çalışmamızda üst seviyelerde uyku endoskopisinin avantajlarını destekleyen ve öğreten veriler elde edilmiştir. Uyku endoskopisi üst hava yollarının değerlendirilmesinde daha faydalı bilgiler verir.

Evaluation of upper airway collapsibility in obstructive sleep apnea patients presenting with complaints of snoring

Öz:
Purpose: The aim of this study was to determine the most appropriate diagnostic method for the detection of the site and configuration of obstructed regions for the successful management of patients with obstructive sleep apnea. Materials and Methods: The study included 40 patients with snoring complaints who underwent septoplasty surgery followed by polysomnography. Evaluation was made in respect of age, gender, Epworth Sleepiness Scale, bodymass index (BMI), modified Mallampati scoring, Müller’s maneuver, and sleep endoscopy data. Patients with collapse of >50% in Müller’s maneuver and sleep endoscopy were considered as serious obstructions.Results: No significant difference was observed between the assessments of Mallampati scoring and sleep endoscopy in the laryngeal region. In the comparison of sleep endoscopy and Müller’s maneuver, significant differences were observed in the lateral band, uvulapalatal region and tongue base.Conclusion: Sleep endoscopy is increasingly preferred since it enables the observation of important changes during sleep. The data of the current study support the view that sleep endoscopy is a more advantageous method at the upper levels as useful information is provided for upper airway evaluation.

A randomized comparison of the proximal crescentic osteotomy and rotational scarf osteotomy in the treatment of hallux valgus

Öz:
Objectives: The aim of this study was to compare clinical and radiological results of proximal crescenticosteotomy (PCO) and rotational scarf osteotomy performed in the treatment of hallux valgus.Methods: A total of 57 consecutive patients (60 feet) with symptomatic hallux valgus deformity wererandomly assigned to one of two groups. The PCO group consisted of 22 women and 5 men (30 feet) andthe mean age was 43(±14.5) years. The scarf group consisted of 23 women and 7 men (30 feet) and themean age was 40.9(±12.6) years. Outcomes were assessed by using of preoperative and postoperativeAmerican Orthopaedic Foot and Ankle Society (AOFAS) scores and visual analogue scale (VAS). Weightbearing X-rays were used for radiological evaluation.Results: The mean AOFAS scores improved from 42(±16.2) to 66.7(±13.4) points in PCO group and from36.2(±16.1) to 73.2(±13.5) points in scarf group. The mean pain score improved from 6.3(±1.3) to 2.4(±2)in PCO group and from 6.5(±1.9) to 2.5(±1.3) in scarf group. The mean hallux valgus angle (HVA)decreased from 38.1(±7.1) preoperatively to 23.8(±8.5) at postoperative first year in PCO group, andfrom 36.1(±7.5) preoperatively to 22.2(±7.5) at postoperative first year in scarf group. The meanintermetatarsal angle (IMA) decreased from 17.3(±3.8) preoperatively to 11.8(±3.3) at postoperativefirst year in PCO group, and from 16.2(±2.6) preoperatively to 9.3(±2.4) at postoperative first year inscarf group.When all the patients were assessed together, the relations between preoperative DMAA valuesand postoperative first year HVA (r ¼ 0,327) and IMA (r ¼ 0,399) values were positive but hadlow significance. The HVA and IMA values were increased in both groups at the end of the firstyear when compared to the postoperative sixth week values (p < 0.01 for both groups for bothvalues).Conclusion: The PCO and the rotational scarf osteotomy in the treatment of hallux valgus deformityprovides a satisfactory correction. The clinical and radiological results of both methods are similar.Especially in patients with high preoperative DMAA, an increase in the HVA and the IMA values mayoccur in the first postoperative year when compared to the postoperative sixth week values.

Postoperative severe hypertension in a patient with acromegaly

Öz:
Acromegaly is a chronic disorder characterized by excessive secretion of growth hormone and elevated insulin-like growth factor-1 levels, usually due to a pituitary adenoma. In this patients, hypertension occurs in 18-60%, with a predominance of diastolic blood pressure elevation that increases in prevalence with age. Hypertension is likely due to increased plasma volume, to decreased ANP levels and to insulin resistance and disease duration are coexisting factors likely to increase the risk of hypertension. In this article, we report a case of a female patient with postoperative severe hypertension who presented with acromegaly secondary to a pituitary microadenoma.

Assessment of cutting performance on drilling with different drilling methods of fiber reinforced polymer composites: A Literature Review

Öz:
Fiber reinforced polymer composite (FRPs) materials are widely used for joining bolted, riveted and pinned in aerospace and aircraft industries due to their engineering properties such as high strength, high damping, low weight and low thermal expansion. However, the structure of FRPs is more complicated than metals, the factors during the drilling process such as hole entry and exit delamination, matrix phase distortion and fiber breakage lead to the reduction of the engineering properties of these materials. These materials is stacked with metals such as titanium and aluminum for drilling operation, especially on body-wing connections in the aerospace and aircraft industries. Each stacked materials is required to determine the optimum cutting conditions for precision surface quality due to its different characteristics during drilling. In this study, the results obtained from studies are presented together regarding to the drilling operation of stacked with metals and unibody FRPs in literature.
