Aşamalı çürük tedavisi: Derleme

Öz:
Pulpanın canlı kalması diş dokularının kırılganlığının azalması için kritik önem taşımaktadır. Dolayısıyla, derin dentin çürüklerinin tedavisinde invaziv olmayan yöntemlerin tercih edilmesi pulpa canlılığını korumak amacıyla tavsiye edilmiştir. Diş hekimliği uygulamaları esnasında özellikle derin çürük lezyonları kaldırılırken pulpaekspozu ile karşılaşılması sık rastlanılan bir durumdur. Pulpa dokusunun rejenerasyon yeteneği yüksek olsada direkt pulpakapaklaması yöntemlerinin başarı oranları çok yüksek değildir. Direkt pulpakapaklaması yöntemine alternatif olarak çürük dokunun tamamını kaldırılmadan ilk randevude yumuşak dentinin sadece bir kısmının kaldırılıp kavitenin geçici olarak restore edildiği aşamalı çürük tedavisi uygulanmaktadır. Aşamalı çürük tedavilerinde ileri ekskavasyonlar ikinci randevuda yapılarak pulpanın canlılığını koruması sağlanmaktadır. Aşamalı çürük tedavisi pek çok farklı bekleme süresi (3, 6, 9, 12 ay) ve yöntemler (pulpa koruyucu kullanarak ya da kullanmadan) ile uygulanabilmektedir. Aynı zamanda ilk randevularda farklı geçici materyaller uygulanabilmektedir ve mikroorganizma sayılarındaki değişiklikler laboratuvar ortamlarında incelenerek tedavinin etkinliği üzerine sonuçlar elde edilebilmektedir. Bu derlemede aşamalı çürük tedavisi yöntemi ile ilgili literatürde bulunan pek çok çalışma incelenerek tedavinin prognozu hakkında bilgi edinilmiştir. Aşamalı çürük tedavisinin klinik olarak önemli bir tedavi seçeneği olduğu ve yapılan çalışmalarda başarılı sonuçlar gösterdiği belirlenmiştir.

Stepwise excavation: A systematic review

Öz:
Vitality of the pulp is critical to the reduce the brittleness of dental tissues.Therefore, non-invasive methods for the treatment of deep dentin caries have been recommended to preserve pulp vitality.During dental practice the occurrence of pulp exposures are frequent, especially when deep caries lesions are being removed although the pulp tissue's ability to regenerateis good, the success rates of direct pulp capping methods are not very high. As an alternative to the direct pulp capping method, a caries treatment named stepwise excavation can be applied in which only a part of the soft dentin is removed and the cavity is temporarily restored.In stepwise excavation treatmentsfurther excavations are made on the second appointment to protect the vitality of the pulp.Stepwise excavation treatment can be conducted with many different waiting periods (3, 6, 9, 12 months) and methods (with or without pulp capping material).Additionally, different temporary materials can be applied at the first appointments and the changes in the number of microorganisms are examined in the laboratory environment obtaining the results on the effectiveness of treatment.In this review, many studies in the literature on stepwise excavation treatment have been reviewed and information about the prognosis of the treatment is obtained.It has been determined that stepwise excavation treatment is a clinically important treatment option and that the studies show successful results
