Ortodontik pekiştirme döneminde hasta memnuniyeti ve ağız hijyeninin değerlendirilmesi

Ortodontik pekiştirme döneminde hasta memnuniyeti ve ağız hijyeninin değerlendirilmesi
Öz: Amaç: Bu çalışmanın amacı hastaların aktif ortodontik tedavisinin ardından pasif izlendiği pekiştirme döneminde memnuniyetlerini ve ağız hijenlerini değerlendirmek ve sabit lingualpekiştirme aygıtı ile ilgili bilgilerini ve şikayetlerini ölçmektir.Gereç ve Yöntem: Aktif ortodontik tedavi bir sene geçmişolan 42 bireyin (26 kız, 16 erkek), alt ve üst sabit pekiştirme aygıtının uygulandığı bölgenin yanısıra total oral plak indeksleri(Pİ) ve papiller kanama indeksleri (PKİ) ölçülmüştür. Ayrıca pekiştirme dönemine ait 22-soruluk bir anketi cevaplamaları istenmiştir. Niceliksel verilerin grupiçi karşılaştırmasında Friedman testi ve niteliksel verilerin karşılaştırılmasında Ki-Kare testikullanılmıştır.Bulgular: Hastaların çoğu aktif ortodontik tedavi bitimindeve pekiştirme döneminde diş dizilimlerinden ve gülümseme estetiklerinden çok memnun olduklarını ifade etmişlerdir(p<0,05). Diş diziliminde geri dönüş olduğunu düşünen hastaların anlamlı çoğunluğu bu durumdan kendilerinin (%23,8)sorumlu olduğunu ifade etmişlerdir (p<0,05). Pekiştirme aygıtına alışmalarının kolay olduğunu (%92,9, p<0,05), ağrıya sebep olmadığını (%76,2, p<0,05), konuşmalarını etkilemediğini(%97,6 p<0,05), ağız kokusuna neden olmadığı (%88,1 p<0,05)ve diş temizliğini etkilemediğini ancak daha özenli fırçalamagerektiğini ifade etmişlerdir (%83,3, p<0,05). Total ağız Pİ’i, üstlingual Pİ’inden (p<0,05); üst lingual Pİ de alt lingual Pİ’indenanlamlı derecede daha düşük tespit edilmiştir. (p<0,05). Altlingual bölge PKİ’i, total ve üst lingual PKİ’den anlamlı dereceyüksektir (p<0,05).Sonuçlar: Hastaların büyük kısmı pekiştirme aygıtına alışmanın kolay olduğunu belirtmekte ve pekiştirme dönemine aitbilgilerin sözlü verilmesini yeterli bulmaktadır. Buna karşınhastaların ¼’ü bu bilgilerin yazılı olarak verilmesini daha faydalı olacağını düşünmektedir. Plak birikimi pekiştirme aygıtınına (hem üst hem alt çenede) bağlı olarak artmaktadır. Kanama indeksi ise alt çene pekiştirme aygıt bölgesinde anlamlıartış göstermektedir. Pekiştirme dönemine ait bilgilerin hemsözlü hem de yazılı broşür olarak ve pekiştirme aygıtı bulunan bölgeler için ayrıntılı ağız hijen eğitiminin verilmesi hastamemnuniyetini ve tedavi sonuçlarını artırabilir.

Evaluation of patient satisfaction and oral hygiene during retention period
Öz: Aim: The purpose of the study was to evaluate the satisfaction of the patients in passive orthodontic retention period after the active orthodontic treatment, measure their knowledge and complaints about fixed lingual retainers and assess the patients’oral hygiene. Materials and Methods: The plaque index (PI) and papillary bleeding index (PBI) of the total oral as well as the fixed retainer application regions (Upper and lower lingual) of 42 patients whose active orthodontic treatment was finished 1 year ago were measured. Additionally, they were asked to answer a 22-item questionnaire regarding the retention period. Intragroup comparison of quatitative and qualitative data was performed using Friedman and Chi-Squared Test, respectively. Results: Most of the patients were satisfied with the alignment and smile aesthetics after the active orthodontic treatment as well as retention period (p<0.05). Most of the patients who suffered from relaps stated that themselves (23.8%) were primary responsible for this results. Fixed retainer were easy accepted (92.9%, p<0.05), not painful (76.2%, p<0.05), had no effect on phonation (97.6%,p<0.05), not resulting in odor (88.1%, p<0.05) and had no effect on oral hygiene, but required more careful brushing (83.3% p<0.05). The average total oral PI score was statistically lesser than upper lingual score; whereas the upper lingual score was lesser than lower lingual region (p<0.05). The average lower lingual PBI was higher than the upper lingual and total oral scores (p<0.05). Conclusions: Most of the patients easily accepted fixed lingual retainers and stated that verbal information transfer is sufficient. However, ¼ of the patients considered written informative materials as more useful. Plaque accumulation increased in relation with the fixed retainer (upper and lower jaw). The bleeding index was meaningful higher in the lower fıxed retainer region. Verbal and written information about the retention period and detailed oral hygiene education for retainer regions may enhance patient satisfaction and treatment outcomes.
