A Patient with Coexistent Type 1 Diabetes Mellitus and ANCA-Negative Granulomatosis with Polyangiitis

Öz:
A 17-year old girl admitted to our outpatient unit with hoarseness and shortness ofbreath. Her physical examination showed nasal deformity. Her medical history included the diagnosis of uveitis and type 1 diabetes mellitus (DM) as well as asthma and chronic sinusitis. Diagnostic work-up showed involvement of the upper respiratory tract (saddle nose deformity, chronicsinusitis), laryngo-tracheo-bronchial involvement (subglottic and bronchial stenosis) and pulmonary involvement (pulmonary nodules) leading to a diagnosis of granulomatous polyangiitis(GPA). We report this child on the basis of the fact that coexistence of type 1 DM and GPA hasnever been reported in our country previously.

Öz:
On yedi yaşında kız hasta, ses kısıklığı ve nefes darlığı şikayetiyle polikliniğimize başvurdu. Fizik muayenesinde burun deformitesi olan olgunun geçmiş hikayesinden üveit ve tip I diabetes mellitus tanısının olduğu, astım ve kronik sinüzit tanılarıyla tedaviler aldığı fakat solunum semptomlarının düzelmediği öğrenildi. Fizik muayene ve tetkiklerinde üst solunum yolu tutulumu (semer burun defekti, kronik sinüzit), larengo-trakeobronkial tutulum (subglottik ve bronkial stenoz) ve akciğer tutulumu (pulmoner nodül) tespit edilerek olguya granülomatozisli polianjitis tanısı konuldu. Tip I diabetes mellitus ve granülomatozisli polianjitis birlikteliği ülkemizden daha önce bildirilmemiş olup ilk olması açısından önemlidir.
