Impaired Visual Habituation in Idiopathic Generalized Epilepsy with Photosensitivity Patients

Introduction: In this study, our goal was to analyze further the corticalexcitability levels in idiopathic generalized epilepsy (IGE) patients withand without photosensitivity.Methods: Forty-two patients (16 men and 26 women; mean age 30±8years; range: 18–43 years) with IGE and thirty healthy age-matchedcontrol subjects (15 men and 15 women; mean age 35±3 years; range:20–45 years) were enrolled for the investigation. We investigated thefollowing two groups: 18 subjects with IGE with photosensitivity (IGE+PS),and 24 patients with IGE without photosensitivity (IGE-PS). Patternreversal and potential inter-peak amplitudes, N75-P100 and P100-N145,as well as the corresponding latencies (N75, P100, and N145) for onehundred responses were measured 6 times. A linear regression slope wasused for N75-P100 and P100-N145.Results: Statistical analysis showed difference between groups wherethe IGE+PS had reduced N75-P100 compared to IGE-PS and controls.In IGE+PS group, the amplitude of N75-P100 was drastically reducedreceiving antiepileptic therapy compared to those not receiving anyanti-epileptic treatment (p=0.035).Conclusion: These results show that the IGE+PS group has a differentphotoparoxysmal response phenotype driven by an unknown anddistinct molecular mechanism. Pre-activation cortical excitability may beincreased in IGE+PS patients compared to the IGE-PS or in healthy group.PR-VEP habituation may project the pathophysiological mechanismsunderlying photosensitivity and it may be potential biomarker inpatients with IGE+PS

Fotosensitif İdiopatik Generalize Epilepsi Hastalarında Vizüel Habitüasyon Defisiti

Öz:
Amaç: Bu çalışmada, ışığa duyarlılığı olan ve olmayan idiyopatik generalize epilepsili (IGE) hastalarda kortikal eksitabilite düzeylerini araştırmayı amaçladık. Yöntem: Çalışmaya IGE’li 42 hasta (26 kadın ve 16 erkek, ortalama 30±8 yaş; 18–43 yıl) ve yaşları uyumlu 35 sağlıklı kontrol (15 kadın ve 15 erkek, ortalama yaş 35±3 yıl; 20–45) alındı. Hastaların 18’i fotosensitif IGE (IGE+PS) ve 24’ü non-fotosensitif (IGE-PS) olarak iki grupta değerlendirildi. Patern reversal vizüel evok potansiyel (PR-VEP) tetkiki ile N75-P100 ve P100-N145 inter-pik amplitüdleri ve N75, P100, N145 latansı, 100 yanıt alınarak altı kez ölçüldü. Habituasyonu değerlendirmek için N75-P100 ve P100-N145 pik amplitüdleri lineer regresyon çizgisi eğimi kullanıldı. Bulgular: Üç grup arasında IGE+PS grubunda N75-P100 amplitüdü IGEPS ve kontrollere göre yalnızca ilk blokta düşük amplitüde sahip olup istatiksel olarak anlamlıydı. IGE+PS grubunda, antiepileptik tedavi alan olgularda N75-P100 amplitüdü, anti-epileptik tedavi almayanlara göre anlamlı derecede düşüktü (p=0,035). Sonuç: Bulgularımız IGE+PS hastalarında farklı fotoparoksismal yanıt (PPR) fenotiplerinin IGE’de farklı patofizyolojik mekanizmalarla desteklenebileceğini düşündürmektedir. Aktivasyon öncesi kortikal eksitabilite IGE+PS hastalarına IGE-PS’den veya sağlıklı gönüllülerden daha yüksek olabilir. PR-VEP habitüasyonu, ışığa duyarlılığın altında patofizyolojik mekanizmaları yansıtabilir ve IGE+PS’li hastalarda potansiyel biyomarker olabilir.
