Sex Differences of Migraine: Results of a Nationwide Home-based Study in Turkey

Introduction: The prevalence of migraine was found to be more thanthree-fold higher in women as compared with men, and in additionto differences in prevalence rates, the characteristics and associatedfeatures might also differ between the sexes. The aim of this study was tocompare sex-specific features of migraine and demographic parametersin a nationwide population-based study in Turkey.Methods: Among 5323 subjects, a total of 871 patients who werediagnosed as having definite migraine according to the diagnostic criteriaof the International Classification of Headache Disorders-III (ICHD-III)were included in our study. The demographic characteristics, associatedfeatures, and triggers of migraine were examined with regard to sex.Results: The study group comprised 640 women (73.5%) and 231 men(26.5%), with a female to male ratio of 2.8:1. Attack duration, meanmigraine disability assessment scores (MIDAS), frequencies of nausea,vomiting, osmophobia, vertigo/dizziness, and allodynia were foundsignificantly different between women and men. When we comparedthese parameters between men and postmenopausal women, all theseparameters were still significant except nausea. Odor was statisticallymore frequent as a reported trigger in women, whereas excessive sleepwas a statistically more frequent triggering factor in men. The ratesof depression and allergy were significantly higher in women whencompared with men.Conclusion: Longer attack duration, higher MIDAS scores, and thefrequencies of nausea, vomiting, osmophobia, vertigo/dizziness,and allodynia were more significant in women and this variance insex persisted after menopause. Also, some trigger factors and comorbidities differed between the sexes. These findings might result fromcomplex genetic factors besides sociocultural influences, biologic, andsociocultural roles. Future studies should continue to explore biologicand genetic factors with respect to sex in migraine.

Öz:
Amaç: Migren prevalansı kadınlarda erkekler ile kıyaslandığında üç kattan fazla oranda sık bulunmuştur ve prevalanstaki farklılığa ek olarak, migren ile ilişkili faktörler ve karakteristikleri de cinsiyetler arasında farklılık göstermektedir. Bu çalışmanın amacı Türkiye genelinde popülasyon tabanlı bir çalışmada migrenin cinse özgü özelliklerini ve demografik parametreleri karşılaştırmaktı. Yöntem: 5323 kişi arasından Baş Ağrısı Bozukluklarının Uluslararası Sınıflaması-III’ün (ICHD-III) tanı kriterlerine göre kesin migren tanısı alan toplam 871 hasta çalışmaya alındı. Demografik özellikler, ilişkili faktörler ve migrenin tetikleyicileri cinsiyete göre incelendi. Bulgular: Çalışma grubu 640 kadın (%73,5) ve 231 erkekten (%26,5) oluşmakta olup, kadın-erkek oranı ise 2,8:1 idi. Atak süresi, ortalama ‘Migraine Disability Assessment Scale’ (MIDAS) skorları, bulantı, kusma, osmofobi, vertigo/dizziness ve allodini sıklığı kadın ve erkek arasında anlamlı olarak farklı bulundu. Bu parametreleri erkek ve postmenopozal kadınlar arasında karşılaştırdığımızda, mide bulantısı dışında tüm bu parametreler hala anlamlıydı. Koku kadınlarda istatistiksel olarak daha sık bir tetikleyici faktör olarak bildirilmişken, aşırı uyku erkeklerde istatistiksel olarak daha sık tetikleyici faktördü. Kadınlarda depresyon ve alerji erkeklere göre anlamlı olarak daha sıktı. Sonuç: Daha uzun atak süresi, daha yüksek MIDAS skorları ve bulantı, kusma, osmofobi, vertigo/baş dönmesi ve allodini sıklığı kadınlarda daha fazla idi ve bu cinsiyet farklılığı menopoz sonrası devam etmekteydi. Ayrıca, bazı tetikleyici faktörler ve komorbiditeler cinsiyetler arasında farklılık gösterdi. Bu bulguların, sosyokültürel etkiler, biyolojik ve sosyokültürel rollerin yanı sıra karmaşık genetik faktörlerden kaynaklanabileceği düşünüldü. Gelecek çalışmalarla cinsiyete göre migrende farklılığa yol açan biyolojik ve genetik faktörlerin araştırılmaya devam edilmesi uygun olacaktır.
