Özel saklama koşulu gerektiren veya soğuk zincire tabi ilaçlar ve uygulamalar açısından değerlendirmeler

Öz:
Soğuk zincir; sıcaklığa hassas bir tıbbi ürününhammadde aşamasından ambalajlı nihai ürünedönüşüp kullanıcıya ulaşıncaya kadar geçen sürederuhsat sahibinin öngördüğü onaylanmış sıcaklıkaralıkları içerisinde kalmasını sağlayan, depolama,taşıma ve dağıtımında uygulanan özel saklamakoşuludur. Soğuk zincir uygulamasına tâbi ürünler;immünomodülatörler, monoklonal antikorlar,hormonlar, enzimler, büyüme faktörleri, plazminojenaktivatörleri, koagülasyon faktörleri ve immünolojikürünler şeklinde sınıflandırılabilir. Soğuk zinciruygulamalarının biyofarmasötik ürünün endüstrideüretiminden, dağıtımına ve uygulanmasına kadar heraşamasında sağlanması gerekmektedir. Soğuk zincirkırılması durumunda ürünlerin uygulanması hastasağlığı için risk oluştururken, bu ürünlerin bozularakatılması, üretiminde kullanılan hammaddelerinve yöntemlerin maliyetli olması nedeniyle ülkeekonomisine zarar vermektedir. Biyofarmasötikürünlerin, yapısında yer alan protein(lerin) kararsızlığınedeniyle soğuk zincir uygulamasına ihtiyaç duyduğusaptanmıştır. Bu derlemede; biyolojik ve biyoteknolojikürünlerin taşınması, saklanması ve dağıtımı sırasındaaktivite kaybı yaşanmaması ve ürün kalitesininher aşamada stabil kalması için uygulanan soğukzincir koşulları kapsamlı olarak değerlendirilmiştir.Ayrıca, biyofarmasötik ürünlerin temel yapısı olanproteinlerin degredasyon yolları ve degredasyonaneden olma koşullarının preparatlar üzerindekietkileri incelenmiş, donma ve sıcaklığa maruziyetinprotein stabilitesi üzerindeki etkisi değerlendirilmiştir.Bu kapsamda aşılardaki soğuk zincir uygulamaları,aşıların nakli ve saklanma koşulları açısından verilenörneklerle değerlendirilmiştir. Bunu takiben soğukzincir yönetimi ile ilgili uluslararası düzenlemeler,kılavuzlar ve Türkiye’deki soğuk zincir uygulamalarınayer verilmiştir.

Öz:
Cold chain; is a special storage condition applied in the storage, transportation and distribution, which ensures that the temperature-sensitive medicinal product remains within the approved temperature ranges prescribed by the registration holder from the raw material stage to the packaged final product until reaching the user. Products subject to cold chain application can be classified as; immunomodulators, monoclonal antibodies, hormones, enzymes, growth factors, plasminogen activators, coagulation factors, immunological products. Cold chain applications determined by the authorities need to be supplied at every stage ranging from the industrial production, distribution and administration of biopharmaceutical products. While the application of products in case of cold chain breakage poses a risk to the patient health, the disruption of these products causes pecuniary damage to the country economy because of the use of expensive raw materials and methods in the production of these products. In this review, the cold chain conditions applied to biotechnological and biological products to avoid the loss of activity and to provide stability at every stage during transport, storage and distribution have been extensively assessed. Unlike synthetic drugs, biopharmaceutical products were found to require cold chain application due to protein instability. In addition, the degradation pathways of proteins, the basic structure of biopharmaceutical products, and the effect of degradation causing conditions on preparations were examined and the effect of exposure to freezing and exposure to temperature on protein stability was evaluated. In this context, cold chain applications have been evaluated with the examples given for transport and storage conditions of the vaccines. Following this, international regulations and guidelines on cold chain management and cold chain applications in Turkey have been included.
