Düşük doz lazer tedavisinin sagittal split ramus osteotomisi sonrası meydana gelen duyu değişiklikleri üzerine etkisi

Öz:
Amaç: Mandibulayı etkileyen dentofasiyal deformasyonlardamandibulanın doğru pozisyona getirilmesi içingeliştirilmiş􀡤 farklı osteotomiler bulunmaktadır. Bunlararasında en sık tercih edilen yöntem sagital split ramusosteotomisidir. Bu tedavi sonrası istenmeyen nörosensoriyalbozukluklar oluşmaktadır. Bu bozukluklar; dişlerde,gingivada, alt dudak ve bazen de çene ucundaparestezi, dizestezi, hipostezi şeklinde görülmektedir.Düşük doz lazer tedavisi literatürde biyomodülatör etkiüreten bir tedavi yöntemi olarak tanımlanmış olup nörosensoriyalbozukluklarda alternatif tedavi seçeneğiolarak popülarite kazanmaktadır. Çalışmamızda düşükdoz lazer terapisinin, bilateral sagittal split ramus osteotomisisonucu oluşan nörosensoriyal değişiklikler üzerineetkisinin incelenmesi amaçlanmıştır.Gereç ve Yöntem: Bu çalışmaya 5’i kadın 6’sı erkek, ortalamayaşları 24 (Dağılım aralığı 17- 30 yıl) olan toplam11 birey dahil edilmiştir. Bu çalışmaya katılan hastalarınrastgele seçilen bir tarafına ameliyattan sonra 24, 48 ve72. saatlerde ve haftada iki kez olmak üzere 3 hafta süreyledüşük doz lazer tedavisi uygulanmıştır. Aynı hastanındiğer tarafına ise plasebo etkisi yaratmak amacıyla lazerprobu cihaz kapalıyken uygulanmıştır. Nörosensoriyaldeğişiklikler aynı cerrah grubundan ameliyat öncesinde,ameliyattan hemen sonra 15. gün, 30. gün, 2. ay ve 6.ayda pamuk pelet testi ve iki nokta ayırım testi kullanılarakdeğerlendirilmiştirBulgular: Plasebo ve uygulama taraflarına ait skor ortalamalarıarasında anlamlı farklılık bulunmamıştır. 6. aydaher iki grupta nörosensoriyal değişikliklerin tamamındaiyileşme görülmüştür.Sonuçlar: Bu çalışmanın sonuçlarına göre nörosensoriyaldeğişiklikleri değerlendirmede kullanılan testlerinsonuçlarına düşük doz lazer terapisinin istatistikselolarak anlamlı etkisi bulunmamaktadır. Bununla beraberpsikolojik açıdan iyileşme sürecinde olumlu etkileri gözönünde bulundurulduğunda basit, ekonomik ve non invazivbir teknik olan düşük doz lazer tedavisinin, bilateralsagittal split osteotomisi sonrası standart bir tedavi protokolühalinde uygulanması önerilebilir.

Effect of low level laser therapy on neurosensorial changes after orthognatic surgery

Öz:
Aim: There are various surgical methods to correct mandibular deformities. One of the most used method is sagittal split ramus osteotomy. Undesirable neurosensorial disorders like paresthesia, dysesthesia, hiposthesia of the teeth, gingiva, chin and lips may occur after this surgical treatment. Low level laser therapy is an alternative, popular and helpful theraphy in treatment of neurosensorial disorders by producing biomodulatory effect. In our study, we evaluated the effect of low level laser theraphy on the neurosensorial changes after orthognatic surgery. Material and Method: 11 patients (5 woman and 6 male ), where the mean age is 24 ( 17-30 years) were included in this study. One randomly selected side of the patients underwent low-level laser treatment at 24, 48 and 72 hours after surgery, and twice weekly for 3 weeks. On the other side of the same patient, the laser probe was applied with the device switched off to create a placebo effect. Neurosensory changes were evaluated by the same surgeon preoperatively, immediately after surgery using cotton pellet test and two-point discrimination test at 15th day, 30th day, 2nd month and 6th month. Results: No significant difference was found between the mean scores of placebo and administration sides. At 6 months, both groups showed improvement in all of the neurosensorial changes. Conclusion: According to the results of the study, no additional treatment is needed to facilitate the healing process of neurosensory changes, but considering the positive effects of the psychological healing process, low-dose laser treatment, which is a simple, economical and non-invasive technique after BSSRO, may be recommended as a standard treatment protocol.
