Recurrent Admissions to Psychiatric Emergency Service: What are the Needs of the Elderly in This Area Differing from Young People and What Can be Done? A Retrospective Comparative Study

Öz:
Objective: Recurrent admissions to psychiatric emergency services (PES) are a multidimensional clinical situation. In this situation, elderly people havedifferent properties and needs compared to younger people. The aim of the present study was to determine the characteristics and needs of the elderlypeople with recurrent admissions to PES in terms of sociodemographic, clinical characteristics and clinical approach, according to the different featuresof the youth, and to draw attention to what can be done.Methods: The files of the patients aged 18 years and over, who had two or more admissions to our hospital’s PES between January 2011 and January2012, were examined by random method (one in 23 patients). A total of 324 patients were included in this retrospective comparative study. The patientswere divided into two groups as those aged 65 years and older (elderly group, n=167) and those aged 18-65 years (young group, n=157) and comparedas per the findings obtained.Results: As per the young people, the elderly often applied to the PES with family members; the rate of physical disease was higher; more physicalexaminations and consultations were requested during the evaluation of the elderly; visual hallucination, memory impairment, and verbal violence werefound to be higher in the symptom profiles. The most common diagnoses of the elderly were determined to be psychosis, behavioral and psychologicalsymptoms related to dementia, bipolar mood disorder, depression and anxiety disorder, and the hospitalization was found to be lower in the elderly.Conclusion: Elderly patients differ from young people in terms of physical disease comorbidity, physical examination, consultation request, symptomprofile, diagnosis and treatment plan. The evaluation of the elderly patient will provide a multidisciplinary approach, the formation of professional teamsin the field should be the main purpose. In this way, recurrent admissions of elderly people to PES can be prevented.

Öz:
Amaç: Psikiyatri acil servislerine (PAS) tekrarlayan başvurular, çok yönlü değerlendirmeyi gerektiren bir klinik durumdur. Bu alanda, yaşlılar gençlere göre farklı özellik ve ihtiyaçlara sahiptir. Bu çalışmanın amacı, PAS’ye tekrarlayan başvuruları olan yaşlıların, sosyodemografik, klinik karakteristikler ve klinik yaklaşım açısından, gençlerden farklı olarak ön plana çıkan özelliklerini, ihtiyaçlarını belirlemek ve neler yapılabileceğine dikkat çekmektir. Yöntemler: Hastanemiz PAS’ye Ocak 2011-Ocak 2012 tarihleri arasında iki veya daha fazla başvurusu olan 18 yaş ve üzerindeki hastaların dosyaları, rastgele yöntemle (23 hastada bir) incelenmiştir. Bu geriye dönük karşılaştırmalı çalışmaya 324 hasta dahil edilmiştir. Hastalar, 65 yaş ve üzeri (yaşlı grup, n=167) ve 18-65 yaş arası (genç grup, n=157) olmak üzere iki gruba ayrılarak, elde edilen bulgular açısından karşılaştırılmıştır. Bulgular: Gençlere göre yaşlıların PAS’ye çoğunlukla aile üyeleri eşliğinde başvurduğu, fiziksel hastalık oranının daha yüksek olduğu, yaşlıları değerlendirme sürecinde daha çok tetkik ve konsültasyon istendiği, semptom profillerine bakıldığında görsel varsanı, bellek bozukluğu ve sözel şiddetin daha yüksek oranda olduğu saptanmıştır. Yaşlılara, en sık konulan tanıların; psikoz, demansa bağlı davranışsal ve psikolojik semptomlar, bipolar duygudurum bozukluğu, depresyon ve anksiyete bozukluğu olduğu ve yaşlıların hastaneye yatırılma oranının daha düşük olduğu belirlenmiştir. Sonuç: Yaşlı hastalar gençlerden fiziksel hastalık eş tanısı, tetkik ve konsültasyon istemi, semptom profili, tanıları ve tedavi planı açısından farklılıklar göstermektedir. Yaşlı hastayı değerlendirme, tanı, tedavi planı, PAS’den çıkış kararı ve çıkış sonrası sürecin planlanmasında multidisipliner yaklaşımı sağlayacak, alanında profesyonel ekiplerin oluşturulması temel hedef olmalıdır. Bu sayede yaşlıların PAS’ye tekrarlayan başvuruları önlenebilir.
