Erişkinlerde COVID-19: Klinik Bulgular

Öz:
Koronavirüs hastalığı 2019 (COVID-19), ilk defa Çin’de tanımlanan, patojenitesi oldukça yüksek zoonotik viral birenfeksiyondur. COVID-19 asemptomatik enfeksiyon, hafif üst solunum yolu hastalığı, şiddetli viral pnömoni kaynaklı solunum yetmezliği ve hatta ölümü kapsayan oldukça geniş bir klinik spektruma sahiptir. Dünya Sağlık Örgütü,Çin’e ait COVID-19 raporuna göre fatalite hızını %3,8 olarak bildirmiştir. Çin’de 72 314 olgunun dahil edildiği çalışmada, olguların %81’i hafif, %14’ü ciddi, %5’i kritik olarak değerlendirilmiş, dokuz yaş ve altındaki grupta ölümbildirilmemiştir. Ancak 70-79 yaş arası vakalarda vaka-ölüm oranı %8.0 ve 80 yaş ve üstü vakalarda %14.8’dir.Çocuklarda COVID-19’un belirti ve bulgularının yetişkinlere benzer veya yetişkinlere göre genellikle daha hafif olduğu bildirilmiştir. T.C. Sağlık Bakanlığı güncel rehberinde gebe olmanın ağır seyirli COVID-19 için ek risk yaratmadığıbildirilmiştir. Hastalığa ait baskın semptomlar ateş ve öksürük olmakla birlikte hastalarda baş ağrısı, konjunktivalhiperemi, nazal konjesyon, boğaz ağrısı, sekresyon artışı, balgam, halsizlik, hemoptizi, nefes darlığı, bulantı-kusma,diyare, karın ağrısı, miyalji, döküntü, tat ve koku duyusu bozukluğu gözlenebilir. Hipertansiyon, kardiyovaskülerhastalık, diyabetes mellitus, kronik akciğer hastalığı, başta hematolojik olmak üzere maligniteler, immunsupresiftedavi veya hastalık, organ nakli, kronik böbrek yetmezliği, obezite, sigara kullanımı mortalite açısından risk faktörleri arasındadır. Lenfosit sayısında ve fibrinojen düzeyinde azalma ve karaciğer-böbrek fonksiyon testleri, troponinI, D-Dimer, laktat dehidrogenaz, protrombin zamanı, kreatinin kinaz, C-reaktif protein, ferritin, interlökin-6 yükseklikleri kötü prognoz belirleyicileri olarak klinisyene yol gösterebilir. Bu derlemenin amacı, güncel literatürlerle erişkinlerde COVID-19 enfeksiyonun klinik bulgularını değerlendirmektir.

Coronavirus disease 2019 (COVID-19) is a zoonotic viral infection that has been identified for the first time in China with a high pathogenicity. COVID-19 has a wide clinical spectrum including asymptomatic infection, mild upper respiratory disease, respiratory failure due to severe viral pneumonia, and even death. World Health Organization reported the fatality rate as 3.8% according to COVID-19 report related to China. In the study, in which 72 314 cases were included in China, 81% of the cases were evaluated as having mild, 14% as serious and 5% as critical illnesses, and no deaths were reported in the age group of nine and under. However, the case-fatality rate was 70% for cases 70-79 years old and 14.8% for cases 80 and older. It has been reported that the signs and symptoms of COVID-19 disease in children are similar to adults or generally milder than adults. It has been reported in the current guideline of the T.R. Ministry of Health that pregnancy does not pose any additional risk for severe course COVID-19 disease. Although the main symptoms of the disease are fever and cough, patients may have headache, conjunctival hyperemia, nasal congestion, sore throat, increased secretion, sputum, malaise, hemoptysis, shortness of breath, nausea, vomiting, diarrhea, abdominal pain, myalgia, rash, taste and smell disorder. Hypertension, cardiovascular disease, diabetes mellitus, chronic lung disease, all malignancies especially hematological, immunosuppressive therapy or disease, organ transplantation, chronic kidney failure, obesity, smoking are among the risk factors for mortality. Decrease in lymphocyte and fibrinogen count and increased levels of liver-kidney function test results, troponin I, D-Dimer, lactate dehydrogenase, prothrombin time, creatinine phosphokinase, C-reactive protein, ferritin, interleukin-6 may lead the clinician as poor prognosis markers. The aim of this review is to evaluate the clinical findings of COVID-19 infection in adults with current literature.
