Koroner Arter Cerrahisinde Yoğun Bakımda Kalış Süresini Etkileyen Faktörler

Öz:
Amaç: Koroner arter baypas greft (KABG) cerrahisi sonrası yoğun bakım kalış sürelerinin uzamasımorbidite, mortalite ve hastane maliyetlerinin artması ile ilişkilidir. Bu çalışmanın amacı KABGoperasyonları sonrası postoperatif yoğun bakım kalış süresinin (YBKS) uzamasıyla ilişkili risk faktörlerini analiz etmektir.Yöntem: Elektif olarak KABG operasyonu geçirecek 199 hasta çalışmaya alındı. Uzamış yoğunbakım kalış süresi 48 saat olarak belirlendi. Hastaların preoperatif demografik verileri, krosklemp ve pompa süreleri, inotrop kullanımları, >2 ünite kan transfüzyonu, hipotermi, entübasyonsüresi, aritmi, alveolo-arteriyel oksijen gradienti (PAO2-PaO2) ve parsiyel arteriyel oksijen/ fraksiyone ve inspire oksijen (PaO2/FiO2) oranları kaydedildi. Yoğun bakımda kalış süresine göre hastalar iki gruba ayrıldı. Grup 1: Yoğun bakım kalış süresi 48 saatten kısa olan hastalar, Grup 2: Yoğunbakımda kalış süresi 48 saatten uzun olan hastalar. İstatik yöntemi olarak Mann Whitney U testive eşli olmayan t testi kullanıldı. Lineer regresyon analizi ile uzamış yoğun bakımla ilişkili faktörleraraştırıldı.Bulgular: Yoğun bakımda kalış süresi Grup 1‘de 26.4±9.1 iken, Grup 2 hastalarında 137.8±72.3saat olarak bulundu. Ventilasyon süresi ise grup 1’de 11.1±4.1 saat, Grup 2’de 74.5±81.0 olarakhesaplandı. Lineer regresyon analizine göre entübasyon süresi, kronik obstrüktif akciğer hastalığı(KOAH), yaş >60, kreatinin > 1.3 mg/dl ve ejeksiyon fraksiyonu (EF) < %45 uzamış yoğun bakımdakalış süresi ile ilişkili faktörler olarak bulundu.Sonuç: Tespit edilen risk faktörlerinin varlığında uygun tedavi seçenekleri ile hastaların desteklenerek operasyon zamanlamasının iyi belirlenmesi, yoğun bakım kalış süresi azaltacaktır

Factors Affecting Intensive Care Unit Stay in Coronary Artery Surgery

Objective: Prolonged ICU stay after coronary artery bypass surgery (CABG) is associated with increased morbidity, mortality and hospital costs. The aim of this study is to analyze the risk factors associated with the prolongation of postoperative intensive care unit stay (ICUS) after CABG operations. Methods: A total of 199 patients who would electively undergo CABG operation were included in the study. Prolonged ICU stay was determined as 48 hours. Preoperative demographic data, cross-clamping and pump times, inotropic uses, >2 unit blood transfusion, hypothermia, intubation time, arrhythmia, alveolo-arterial oxygen gradient (PAO2 -PaO2 ) and partial arterial oxygen / fractional inspired oxygen (PaO2/FiO2) ratios were recorded. Patients were divided into two groups according to the length of stay in the intensive care unit as shorter (Group 1), and longer (Group 2) than 48 hours. Mann-Whitney U , and unpaired t tests were used as statistical methods. The factors associated with prolonged intensive care stay were investigated by linear regression analysis. Results: Duration of stay in intensive care unit was 26.±9.1 hours in Group 1, and 137.8±72.3 hours in Group 2 patients. Ventilation time was calculated as 11.1±4.1 hours in Group 1 and 74.5±81.0 hours in Group 2. According to the linear regression analysis, intubation time, chronic obstructive pulmonary disease (COPD), age >60 years, creatinine >1.3 mg/dl and ejection fraction (EF)<45% were found to be factors associated with prolonged ICU stay. Conclusion: In the presence of identified risk factors, improving the patients’health with appropriate treatment options, and specifying the ideal timing of the operation will shorten ICU stay.
