Kronik Total Oklüzyonlu Hastalarda Monosit/Yüksek Yoğunluklu Lipoprotein Oranı ile Koroner Kollateral Dolaşım Arasındaki İlişki

Öz:
Amaç: Koroner kollateral dolaşım koroner arter hastalığı olan hastalarda iskemik olan bölgeye kan sunumunun alternatif bir kaynağıdır.Bu çalışmada amacımız, kronik total oklüzyonlu hastalarda monosit/ yüksek yoğunluklu lipoprotein oranı ile koroner kollateral dolaşımgelişmişlik düzeyi arasındaki ilişkiyi araştırmak.Gereç ve Yöntem: En az bir ana koroner arterinde tam tıkanıklık saptanan stabil koroner arter hastalıklı 226 hasta çalışmaya retrospektif olarak dahil edildi. Koroner kollateral dolaşım, Rentop-Cohen sınıflamasına göre sınıflandırıldı. Grade 0 ve 1 kollateral az, grade 2 ve3 kollateral iyi gelişmiş koroner kollateral dolaşım olarak kabul edildi.Bulgular: Çalışmaya 226 hasta alındı. 76 hastada (65.55±8.78 yıl, %82.9 erkek) az gelişmiş koroner kollateral dolaşım saptanırken, 150hastada (66.41±9.28 yıl, %82.7 erkek) iyi gelişmiş koroner kollateral dolaşım saptandı. İskemik semptomların süresi iyi gelişmiş koronerkollateral dolaşımlı grupta daha uzundu. Eritrosit dağılım genişliği 15’e (13,72-16,57) karşı 14,50 (13.60-16.00), p=0,024) az gelişmiş koroner kollateral dolaşımlı grupta iyi gelişmiş koroner kollateral dolaşımlı gruba göre daha yüksek bulundu. Ayrıca, monosit (0,61 (0,50-0,81) 103/µL’ye karşı 0,59 (0,41-0,74) 103/µL, p=0,024) ve monosit/yüksek yoğunluklu lipoprotein oranı (16,27’e (12,34-22,05) karşı 14,99(10,99-20,79), p=0,031) iyi gelişmiş koroner kollateral dolaşımlı grupta az gelişmiş koroner kollateral dolaşımlı gruba göre daha yüksekti. Çok değişkenli lojistik regresyon analizinde iskemik semptomların süresi, monosit/ yüksek yoğunluklu lipoprotein oranı ve eritrosit dağılım genişliği düzeyi koroner kollateral dolaşım gelişmişlik düzeyi ile ilişkili bulundu.Sonuç: Artmış Enflamasyonun belirteçleri olan monosit/yüksek yoğunluklu lipoprotein oranı ve eritrosit dağılım genişliği düzeyi, bağımsız bir şekilde koroner kollateral dolaşım gelişimiyle ilişkilidir

The Relationship Between Coronary Collateral Circulation and Monocyte to High Density Lipoprotein Ratio in Patients with Chronic Total Occlusion

Öz:
Objective: Coronary collateral circulation constitutes an alternative route for blood flow to the ischemic myocardium in patients with significant epicardial coronary artery stenosis. In this study, we aimed to investigate the relationship between coronary collateral circulation and monocyte to high density lipoprotein ratio in patients with chronic total occlusion. Material and Method: Patients who had chronic total occlusion in at least one major coronary artery, were included in this retrospective study. Coronary collateral circulation was graded according to Rentrop-Cohen classification. Grade 0 and 1 collaterals were classified as poor coronary collateral circulation; Grade 2 and 3 collaterals were classified as good coronary collateral circulation. Results: 226 patients were included in the study. While 76 patients (65.55±8.78 years, 82.9% male) were in the poor coronary collateral circulation group, 150 patients (66.41±9.28 years, 82.7% male) were in the good coronary collateral circulation group. Duration of ischemic symptoms was longer in the good coronary collateral circulation group. Red blood cell distribution width values were significantly higher in patients with poor coronary collateral circulation than in those with good coronary collateral circulation (15 (13.72- 16.57) vs 14.50 (13.60-16.00), p=0.024, respectively). Also, monocyte counts (0.61 (0.50-0.81) 103/µL vs. 0.59 (0.41-0.74) 103/µL, p=0.024, respectively) and monocyte to high density lipoprotein ratio (16.27 (12.34-22.05) vs. 14.99 (10.99-20.79), p=0.031, respectively) were significantly higher in patients with good coronary collateral circulation than in those with poor coronary collateral circulation. Multivariate regression analysis revealed that duration of ischemic symptoms, red blood cell distribution width and monocyte to high density lipoprotein ratio were independently associated with better developed coronary collateral circulation . Conclusion: In this study, we demonstrated an independent relationship between monocyte to high density lipoprotein ratio, red blood cell distribution width level and coronary collateral circulation development in patients with chronic total occlusion.
