Yaşa Bağlı Maküla Dejenerasyonunda Subretinal Hiperreflektif Materyalin Prognostik Rolü

Öz:
Amaç: Neovasküler tip yaşa bağlı maküla dejenerasyonunda spektral domain optik koherans tomografide tespit edilen subretinal hiperreflektif materyal ile başlangıç ve intravitreal tedavi sonrası 24. aydaki görme keskinliği arasındaki ilişkiyi değerlendirmek. Gereç ve Yöntem: 2016 ile 2019 yılları arasında daha önce tedavi almamış kliniğimizde neovasküler tip yaşa bağlı maküla dejenerasyonu tanısı alan ve üç yükleme dozu sonrası pro re nata antivasküler endotelyal büyüme faktörü tedavisi uygulanan hastalar geriye dönük olarak incelendi. Başlangıç ve 24. aydaki spektral domain optik koherans tomografi kesitlerindeki subretinal hiperreflektif materyal varlığı ve morfolojik özellikleri (maksimum uzunluk, alan, reflektivite, sınır özelliği ve homojenite) neovasküler aktivite (intraretinal sıvı, subretinal sıvı ve santral maküla kalınlığı), eksternal limitan membran ve ellipsoid zon integritesi değerlendirildi. Bu parametreler ile Snellen sıra değişimi arasındaki ilişki analiz edildi. Bulgular: 108 hastanın 108 gözü çalışmaya dahil edildi. Çalışmaya katılan hastaların 76’sında (%70,4) ilk muayenede; 42’sinde (%38,9) 24. ayda subretinal hiperreflektif materyal tespit edildi. Yaş ortalaması 71,5±9,1 yıl ve ortalama intravitreal enjeksiyon sayısı 7,5±4,9 idi. Başlangıç muayenedeki daha geniş subretinal hiperreflektif materyal varlığı ile 24. aydaki nonhomojen subretinal hiperreflektif materyal varlığı daha fazla Snellen sıra kaybı ile ilişkiliydi. Sonuç: Bu çalışma spektral domain optik koherans tomografi ile subretinal hiperreflektif materyal analizinin neovasküler tip yaşa bağlı maküla dejenerasyonunda prognostik morfolojik bir belirteç olabileceğini desteklemektedir.

Prognostic Role of Subretinal Hyperreflective Material in Age-Related Macular Degeneration

Öz:
Objective: To evaluate the relationship between subretinal hyperreflective material detected in spectral domain optical coherence tomography and visual outcomes at baseline and 24th month visit after intravitreal treatment in neovascular age-related macular degeneration. Material and Method: Between 2016 to 2019 treatment naive patients diagnosed with neovascular age-related macular degeneration in our clinic, treated with 3 monthly intravitreal anti vascular endothelial growth factor treatment, continued pro re nata were reviewed retrospectively. On spectral domain optical coherence tomography scans at baseline and 24th month visit, presence and morphologic features of subretinal hyperreflective material (maxium length, area, reflectivity, border properties and homogenity), neovascular activity (intraretinal fluid, subretinal fluid, central retinal thickness) and integrity of external limitan membrane and ellipsoide zone were identified. Correlation with these parameters and change in Snellen line were analysed. Results: One hundred eight eyes of 108 patients were recruited in the study. Of the patients enrolled in the study 76 (70.4%) had subretinal hyperreflective material at baseline and 42 (38.9%) had subretinal hyperreflective material at 24th month visit. Mean age was 71.5±9.1 years and mean number of intravitreal injection was 7.5±4.9. Broader subretinal hyperreflective material at first visit and nonhomogenity of subretinal hyperreflective material at 24th month were correlated with more decrease in Snellen line. Conclusion: This study supports the analysis of subretinal hyperreflective material by spectral domain optical coherence tomography may be a prognostic morphologic biomarker in neovascular age-related macular degeneration.
