Tam kat rektal prolapsus cerrahi tedavisinde rektopeksi ve rektopeksi ile birlikte sigmoid rezeksiyonun karşılaştırılması

Öz:

Amaç: Bu çalışmada amaç rektal prolapsus nedeniyle uygulanan cerrahi tedavide sigmoid rezeksiyonun gerekliliğini değerlendirmektir. Yöntem: Ocak 2000 ile Ocak 2007 tarihleri arasında rektal prolapsus nedeniyle cerrahi tedavi uygulanarak rektopeksi ya da rektopeksi ile birlikte sigmoid rezeksiyon uygulanan 31 hasta çalışmaya alınmıştır. Hasta dosyaları geriye dönük olarak taranarak hastalar yaş, cinsiyet, geçirilmiş abdominal cerrahi, cerrahi sonrası komplikasyon, eşlik eden hastalıklar, yineleme, operasyon süresi ve hastanede kalma süresi yönünden karşılaştırıldı.Bulgular: Olguların 18 tanesine sadece sentetik yama ile rektopeksi (Grup I) uygulanırken 13 olguya rektopeksinin yanısıra sigmoid rezeksiyon (Grup II) uygulandı. Ortalama yaş 57 (aralık, 34-76) olup olguların %74'ü kadın idi. Atelektazi ve yara infeksiyonu en sık komplikasyonlardı. Rektopeksi ve rektopeksi ile birlikte sigmoid rezeksiyon uygulanan hastalar arasında gerek medikal gerekse cerrahi komplikasyonlar açısından anlamlı fark saptanmamıştır (P=1.00 ve P=0.09). Her iki grup arasında operasyon süresi yönünden anlamlı fark olmakla birlikte (P=0.001), hastanede kalma süreleri arasında fark saptanmamıştır (P=0.34). Her iki grupta da yineleme ve ölüm saptanmamıştır.Sonuç: Bu çalışma az sayıda hastadan oluşmasına ve bu da çalışmayı kısıtlamasına karşın deneyimlerimiz sigmoid rezeksiyon ile birlikte olsun ya da olmasın rektopeksinin rektal prolapsus tedavisinde güvenli ve etkin bir tedavi olduğunu göstermektedir. Sigmoid rezeksiyon cerrahi sonrası komplikasyonu artırmamaktadır. Bu nedenle sigmoid kolonun normalden uzun olduğu ya da konstipasyon öyküsü olan olgularda uygulamaktan kaçınılmamalıdır.

Comparison of surgical outcome of rectopexy with and without sigmoid resection for fullthickness rectal prolapse

Öz:

Purpose: This study was undertaken to evaluate the necessity of sigmoid resection for rectal prolapse. Materials and Methods: Between January 2000 and January 2007, a total of 31 patients underwent rectopexy with and without sigmoid resection for rectal prolapse. The data were retropectively collected for the age, sex, previous abdominal surgery, morbidity, concomitant disease, recurrence, duration of operation, and length of hospital stay.Results: Rectopexy was performed in 18 patients (Group I), and sigmoid resection plus rectopexy was performed in 13 patients (Group II). The mean age of the patients was 57 (range,34-76) years and 74% were woman. Atelectasis and wound infections were the most frequent morbidities. The medical and surgical complication rates were not significantly different between the rectopexy and rectopexy with resection groups (P = 1.00 and P= 0.09). Although operation time was significantly different (P=0.01), length of hospital stay was not significantly different (P=0.34) among the groups. There were no recurrence or mortality.Conclusion: Although the number of patients is small and this feature restricts the study, our experience indicates that rectopexy with and without sigmoid resection were safe, and effective for the treatment of rectal prolapse. Sigmoid resection did not increase morbidity. It is well suited to patients with a long redundant sigmoid and history of constipation.
