Transüretral prostatektomilerde etanol monitorizasyonu

Öz:
Bu çalışmada, rejyonel anestezi ile transüretral prostatektomi (TURP) geçirecek 30 olguda, irrigasyon sıvısına etanol (%1) eklenerek, solunum sonu etanol konsantrasyonu ile hemodinamik ve biyokimyasal değişiklikler arasındaki ilişkiler değerlendirildi. Preoperatif ve operasyon boyunca; kan basıncı, kalp atım hızı, periferik oksijen satürasyonu (SpO<sub>2</sub>) ve santral venöz basınç (SVB) ölçümleri yapıldı. Ayrıca hemoglobin (Hb), hematokrit (Htc), serum sodyum (S-Na), potasyum (S-K) ve osmolalite bakılmak üzere kan örnekleri alındı. Solunum sonu etanol konsantrasyonu alkolmetre ile onbeş dakika aralarla izlendi. Vital bulgular, intraoperatif ve postoperatif ilk 24 saat normal sınırlarda seyretti. Operasyon sırasında; Hb, Htc, S-Na azaldı ve Hb ile Htc'deki bu azalma istatistiksel olarak anlamlı idi (p<0.05); solunum sonu etanol konsantrasyonu ve SVB arttı (p<0.05). Solunum sonu etanol konsantrasyonu ile Hb (r=-0.82, p<0.05), Htc (r=-0.975, p<0.05)ve S-Na (r=-0.87, p<0.05) arasında negatif, solunum sonu etanol konsantrasyonu ile SVB (r=0.66, p>0.05) ve zaman (r=0.97, p<0.05) arasında ise pozitif ilişki saptandı. Olguların hiçbirinde transüretral rezeksiyon (TUR) sendromu gelişmedi , %1 etanol konsantrasyonu olgular tarafından iyi tolere edildi. Sonuç olarak, TUR sendromunun erken teşhis edilmesinde, irrigasyon sıvısına etanol eklenmesi ile solunum sonu etanol konsantrasyonu izlenmesinin basit, ucuz, hızlı ve güvenilir bir yöntem olduğu kanısına varıldı.

Ethanol monitoring in transurethral prostatic surgery

Öz:
In this study, ethanol concentration in the expired breath was compared with various biochemical and hemodynamic changes after transurethral prostatectomy (TURP) in irrigation fluid added ethanol 1% in 30 patients under regional anesthesia.During the operation blood pressure, pulse rate, peripheral oxygen saturation and central venous pressure (CVP) were measured. Blood samples were drawn to determine hemoglobin (Hb), hematocrite (Htc), serum sodium (S-Na), potassium (S-K) levels and osmolality. Expiratory ethanol concentrations were determined with alcoholmeter every 15 minutes.Vital signs were within normal limits intraoperatively and in the postoperative 24 hours. While there were decreases in Hb (p<0.05), Htc (p<0.05) and S-Na (p>0.05) concentrations during the operations, expiratory ethanol concentration (p<0.05) and CVP (p<0.05) increased. There were negative correlations between expiratory ethanol concentration and Hb (r=-0.82, p<0.05), Htc (r=-0.975, p<0.05) and S-Na (r=-0.87, p<0.05) concentration, but there were positive correlations between expiratory ethanol concentration and CVP (r=0.66, p>0.05) and operative time (r=0.97, p<0.05). No transurethral resection (TUR) syndrome was observed in any patient. One percent ethanol concentration was well tolerated by all patients. As a result, in order to diagnose TUR syndrome earlier, addition of ethanol into the irrigation fluid and determination of expiratory ethanol concentrations in end expiratory air is a simple, cheap, fast and a safe method.
