Yüksek ve düşük akımlı desfluran anestezisinde kognitif fonksiyonların değerlendirilmesi
Öz: Amaç: Kognitif fonksiyon, bilinç düzeyi, hafıza, oryantasyon, konsantrasyon, genel bilgi, zeka, anlama ve yargı yeteneği bileşenlerinden oluşur. Postoperatif kognitif disfonksiyon (POKD), bellek ve konsantrasyondaki yetersizlikle karakterize multifaktoriyel bir tablodur. Hastanın derlenme evresi içinde bilişsel işlevlerinin eski düzeyine gelmesi anestetik maddenin farmakokinetiği, anestezi ve cerrahi yöntem ile ilişkili olmaktadır. Bu çalışmada, düşük ve yüksek akımlı desfluran anestezisi uygulamasında preoperatif ve postoperatif dönemdeki bilişsel işlevlerin incelenmesi amaçlandı.Yöntem: Çalışmaya Lokal Etik Kurul onayı alındıktan sonra ASA I-II, 15-55 yaş arası dahil edilen 32 erişkin olgu, yüksek akım. (Grup I, n=16) ve düşük akım (Grup II, n-16) olmak üzere iki gruba ayrıldı. Premedikasyon yapılmadı. Anestezi indüksiyonu tiyopental 5-7 mg kg'1 ve fentanil 1,5 ]ug kg'1 i.v ile yapıldı ve endotrakeal entübasyon öncesi nöromusküler gevşeme için rokuronyum 0,6 mg kg'1 kullanıldı. Yüksek ve düşük akımlı anestezi grubundaki hastalarda sırasıyla %50 N20+02 4 l dk'1 ve 1 l dk1 akım ile desfluran uygulaması yapıldı. Hastaların derlenmesi 5., 10., 15., 30. ve 60. dakikalarda Modifiye Aldrete Skoru (MAS) ile değerlendirildi. Hastaların kognitif fonksiyonları preoperatif ve postoperatif 1. saatte sırasıyla Sözel İşitsel Bellek Testi (SIBT) ve S troop İnterferans Test (SİT) ile değerlendirildi.Bulgular: Her iki grupta preoperatif ve postoperatif SİBT1, SİBT 5, SİBT 6, SİBT 7, SİBT 8 ve SÎT skorlarında anlamlı bir etkileşim saptanmamıştır. Postoperatif uyanma zamanı ile SİBT 1, SİBT 5, SİBT 6, SİBT 7, SİBT 8, SIT skorları arasında anlamlı bir korelasyon saptanmamıştır.Sonuç: Her iki anestezi uygulaması bilişsel işlevler açısından farklılık göstermemiştir. Postoperatif derlenme zamanı ve bilişsel işlevler arasında ilişki bulunmaması nedeniyle desfluran anestezisindeki taze gaz miktarının kognitif bilişsel fonksiyonları etkilemediğini düşünmekteyiz.

The evalution of cognitive functions in desflurane anesthesia with low and high flow
Öz: Objective: Cognitive function consists of the level of consciousness, memory, orientation, concentration, general knowledge, intelligence, understanding and judgment. Postoperative cognitive dysfunction is a multifactor state which is characterized by disability of memory and concentration. At the recovery stage, the patient's cognitive functions returning to its original level is closely related with pharmacokinetics of the anesthetic material, anesthesia, desfluran used in low and high flow anesthesia. This study aimed at examining cognitive functions in the preoperative ve postoperative period when low and high flow of desfluran anesthesia was applied.Methods: After Institutional Ethics Committee approval, ASA IIII, 32 patients of 15-55 ages two groups. One group (Group I, n=16) had high flow and the other (Group II, n=16) had low flow. Premedication was not applied. Anesthesia was induced with 1.5 jig kg' fentanyl and 5-7 mg kg' thiopental. Endotracheal intubation was performed after 0.6 mg kg' rocuronium.Patients, included in Group I and in Group II were applied 4 L min-1 50 % N20+02 and 1 L min ' 50 % N20+02 respectively. Patient's recovery parameters were evaluated with Modified Aldrete Score (MAS) at 5., 10., 15., 30. and 60. minutes. Cognitive functions of the patients were evaluated with Verbal Auditory Memory Test (VAMT) and Stroop Interference Test (SIT).Results: No significant result was found in the preoperative and postoperative VAMT 1, VAMT 5, VAMT 6, VAMT 7, VAMT 8 and SIT scores between Group I and Group II.No correlation was confirmed between postoperative recovery time and VAMT 1, VAMT 5, VAMT 6, VAMT 7, VAMT 8, SIT score.Conclusion: Neither of the anesthetic techniques showed differences with respect to cognitive functions. We conclude that since no relationship between postoperative recovery time and cognitive functions was found, the amount of fresh gas used in the desfluran did not affect the cognitive functions.
