The incidence of group B streptococcus colonization in the female genital tract

Öz:

Amaç: Bu çalışma, anne ve bebeğinde çeşitli klinik tablolara yol açan Grup B streptokokların (GBS) üretkenlik çağındaki kadınlardan alınan vajinal ve rektal sürüntülerde bulunma sıkIığını; sosyoekonomik düzeyin, jineko-obstetrik öykünün ve aile planlaması yöntemi olarak kullanılan rahim içi araç uygulamasının taşıyıcılıkla ilgisini araştırmak amacıyla yapılmıştır. Yöntem ve Gereçler: Sosyoekonomik düzeyi düşük kadınlarda hizmet veren Ümraniye Ana Çocuk Sağlığı ve Aile Planlaması (AP) Merkezi ile Fikirtepe Kadın Sağlığı ve AP Merkezi polikliniklerimize başvuran 166 kadın ile Marmara Üniversitesi Kadın Hastalıkları ve Doğum Polikliniğine başvuran, ekonomik seviyesi diğerlerine göre yüksek 62' si gebe 121 kadından alınan vajinal ve rektal sürüntülerde kültür yöntemiyle GBS varlığı araştırılmıştır. Bulgular: Genital yoldan izole edilen GBS oranı birinci grupta %12.04 (20 hasta), ikinci grupta ise % 7.43 ( 9 hasta) olarak bulunmuştur. İki sonuç arasındaki fark istatistiksel olarak anlamlı bulunmamıştır. Jinekolojik yakınmanın taşıyıcılıkla ilişkisi sınırda anlamlı olarak değerlendirilmiştir. (p=0.05) Rahim içi araç kullananlarda GBS bulunma sıklığı, kullanmayanlara göre farklı bulunmamıştır. GBS taşıyıcılığının gebelerde gebe olmayanlara göre farklı olmadığı gözlenmiş, gebelik, düşük doğum ve küretaj sayıları ile GBS kolonizasyonu arasındaki ilişkiye bakıldığında, gruplar arasında istatistik i fark saptanmıştır (p<0.05). Tartışma: Genital yolda GBS bulunma sıklığının önceki çalışmaların sonuçlarına benzer şekilde yüksek bulunması, önlemler ve çözümler yönünde araştırılmalar yapılması ve konunun mali boyutlarının tartışılması gereğini düşündürmektedir

Öz:

OBJECTIVE: In this study, we aimed to study the incidence of colonization of Group B Streptococcus (GBS) which can cause significant maternal and neonatal sequela, in reproductive aged women by taking rectal and vaginal swabs. We also wanted to find the association of carrier status with the socioeconomic status, gynecologic-obstetric history and the use of intrauterine device. STUDY DESIGN: A low socioeconomical group was taken from the Ümraniye and Fikirtepe Maternal - Child Health Clinics, a totally of 166 women, and a high socioeconomical group was taken from the Marmara University Hospital Obstetric and Gynecology Clinic, out of 121 women, 62 were pregnant. Rectal and vaginal swabs were taken and these swabs were evaluated at the culture media. RESULTS: Genital (GBS) colonization in the 1st group was 12.04% (20 patients), and in the 2nd group was 7.43% (9 patients). The incidence of GBS colonization was not different between the groups. The relations between the genital GBS colonization and the symptoms were studied and relations were found significant (p=0.05). We did not find any relation between IUD usage and GBS colonization. GBS colonization was not different in pregnant and nonpregnant women. But when we analyzed pregnancy, delivery, abortus and curettage numbers and GBS colonization, we found statistically significant differences between the groups (p<0. 05). CONCLUSIONS: GBS is a bacterium that may cause devastating diseases. The iicidence of group B streptococcal neonatal infections has remained unchanged during the last 20 years, and recent reports indicate that attack rates in nonpregnant adults are substantial. Two basic approaches, chemoprophylaxis and immunoprophylaxis, have been suggested as means by which prevention of group B streptococcal infections might be accomplished.
