Prostat hiperplazisinde transüretral balon dilatasyonun ve transüretral rezeksiyonun karşılaştırılması

Öz:

Bu çalışmamızda transüretral prostatik balon dilatasyon ve transüretral prostatektominin etkinlik ve güvenilirliğini karşılaştırmayı amaçladık. Türkiye Yüksek İhtisas Hastanesi Üroloji Kliniği'nde 26 hastaya dilatasyon ve 26 hastaya da transüretral rezeksiyon yapıldı. Hastalar ameliyatlardan sonra birinci, üçüncü ve altıncı aylarda yeniden değerlendirildi. İki yöntemin karşılaştırılması için uluslararası prostat semptom skoru, maksimal ve ortalama akım hızı, rezidüel idrar miktarı, ameliyat, sonda alınma ve hastanede kalış süreleri, kanama, enfeksiyon, üretral darlık, üriner inkontinans gelişimi ve cinsel fonksiyonlar dikkate alındı. Altı aylık takipte dilatasyon grubunda semptom skoru ortalama %46, artık idrar %14.7 azaldı. Maksimum akım hızı %25.5 ve ortalama akım hızı %31.3 arttı. Rezeksiyon grubunda bu değerler sırasıyla %88.2, %83.5, %117.5 ve %144.1 oldu. Dilatasyon grubunda sağlanan semptomatik ve üroflovmetrik iyileşme hali kısa süre devam etmesine rağmen, rezeksiyon grubunda devamlı oldu. Ameliyat ve üretral sonda alınma süreleri, hematüri ve retrograd ejekülasyon gelişimi açısından dilatasyon daha güvenilirdi. Hastanede kalış süresi, enfeksiyon, darlık, inkontinans ve erektil disfonksiyon gelişimi açısından iki grup arasında fark saptanmadı. Dilatasyonun etkinliği rezeksiyondan daha az ve kısa sürelidir. Ancak yüksek riskli veya nispeten genç hastalarda tercih edilebilecek daha güvenilir bir yöntemdir.

Comparison of transurethral balloon dilatation and transurethral resection in BPH

Öz:

In this study, we aimed to compare the effectiveness and safeness of the transurethral prostatic balloon dilatation and transurethral prostatectomy. Dilatation in 26 patients and transurethral resection in 26 patients were performed at Türkiye Yüksek İhtisas Hospital. Patients were reevaluated at postoperative first, third and sixth months. International prostate symptom score, maximum and average flow rates, residual urine, operation, hospitalization and catheter removal times, bleeding, infection, urethral stricture, urinary incontinence and sexual functions were taken into consideration for comparison of two methods. At 6 months follow-up, in dilatation group, symptom score and residual urine decreased averagely 46% and 14.7%. Maximum and average flow rates increased 25.5% and 31.3%. In resection group, these values were 88.2%, 83.5%, 117.5% and 144.1% respectively. Dilatation was more reliable for operation and urethral catheter removal times, hematuria and retrograd ejaculation. Statistically there were no difference between two groups for the development of the hospitalization time, infection, stricture, incontinence and impotence. The effectiveness of the dilatation is less and short than the resection. However, it is a more reliable method that may be preferred for patients with high-risk or relatively young.
