PREVALENCE OF BILATERAL SYMMETRY IN ROOT AND ROOT CANAL ANATOMY OF MANDIBULAR PREMOLARS: A CONE-BEAM COMPUTED TOMOGRAPHY STUDY

Öz:
Amaç: Bu çalışmanın amacı; birinci ve ikinci alt küçük azı dişlerinde kök ve kök kanal anatomilerinin incelenmesi, aynı hastada karşılıklı simetrinin değerlendirilmesi, hastanın cinsiyeti ile simetri varlığı arasındaki ilişkinin belirlenmesidir. Gereç ve Yöntem: Çift taraflı alt birinci (n=540) ve ikinci küçük azı dişi (n=450) bulunan 495 hastanın (225 erkek, 270 kadın) konik ışınlı bilgisayarlı tomografi görüntüleri incelendi. Hastanın cinsiyeti, kök ve kök kanal sayılarının sayısı, her kök için Vertucci Sınıflandırması ve karşılıklı simetri varlığı kaydedildi. Cinsiyet ile simetri ilişkisi Fisher's kesin olasılık testi kullanılarak belirlendi (P=0.05).Bulgular: Tek kök ve tek kanal varlığının alt birinci ve ikinci küçük azı dişlerinde en sık görülen anatomi olduğu belirlendi. Alt birinci küçük azı dişlerin % 7.5'inin; alt ikinci küçük azı dişlerin ise % 1.3'ünün iki köklü olduğu bulundu. Alt birinci ve ikinci küçük azı dişlerinde birden fazla kanal bulunma sıklığı sırasıyla %15.2 ve %3.1 bulundu. Tek köklü birinci küçük azı dişlerin % 8.6'sının ikinci küçük azı dişlerin ise % 1.8'inin Tip 1'den farklı bir anatomiye sahip olduğu görüldü. Alt birinci küçük azı dişlerde simetri varlığı hastaların % 92.97'sinde gözlenirken alt ikinci küçük azı dişlerde ise bu oran % 97.78 bulundu. Hastanın cinsiyeti ile simetri arasında istatistiksel olarak anlamlı bir farklılık bulunmadı (P>0.05).Sonuç: Alt küçük azı dişlerin hastanın cinsiyeti fark etmeksizin kök ve kök kanal anatomisi açısından yüksek oranda karşılıklı simetriye uyduğu belirlendi. Diş hekimleri alt küçük azı dişlerini tedavi ederken karşılaşacakları anatomik varyasyon varlığında aynı hastanın karşıt dişinde de benzer bir anatominin olabileceğini akılda bulundurmalıdır.

ALT KÜÇÜK AZI DİŞLERİN KÖK VE KÖK KANAL ANATOMİSİNDE ÇİFT TARAFLI SİMETRİ SIKLIĞI: KONİK IŞINLI BİLGİSAYARLI TOMOGRAFİ ÇALIŞMASI

Öz:
Background and Aim: This study was aimed to analyze the root and root canal anatomy of mandibular first and second premolar teeth; to explore the bilateral symmetry in the same patient, and to determine whether there is a correlation between bilateral symmetry and patient's sex.Materials and methods: A total of 495 patients (225 males, 270 females) who have bilateral mandibular first premolar (n=540) or mandibular second premolar (n=450) were analyzed using conebeam computed tomography scanning. Details of sex, the number of roots and canals, Vertucci's Classification of Root Canal Type for each root, and the symmetry were recorded. Data were analyzed by the Fisher exact test (P=0.05).Results: One root with 1 canal was the normal anatomy of mandibular first and second premolars. Overall, only 7.5% and 1.3% of the mandibular first and second premolars had 2 separate roots, respectively. The incidence of mandibular first and second premolars with more than one root canal was 15.2% and 3.1%, respectively. Also, 8.6% of first and 1.8% of second one-rooted premolars had a root canal anatomy other than Type I. Mandibular first premolars were found to be symmetrical in 92.97% of the patients, whereas mandibular second premolars were symmetrical in 97.78% of the patients. No significant correlation was found between the patient's sex and the symmetry (P>0.05).Conclusion: Mandibular premolars showed perfect bilateral symmetry in root and canal morphology regardless of the patient's sex. The high rate of anatomical symmetry should be taken into consideration when treating two opposite premolars in the same patient.
