Deneysel splenik arter ligasyonu ve splenik ototransplantasyonun pnömokok sepsisi oluşumuna etkisi
Öz: Dalak koruyucu cerrahi yöntemlerden splenik arter li¬gasyonu ve splenik ototrânsplantasyonun pnömokok sep¬sisi oluşumuna etkileri ve birbirlerine karşı üstünlüklerini ortaya koymak amacıyla deneysel bir çalışma plan¬lanmıştır. Bu amaçla ağırlıkları 162^270 gr arasında değişen genç ve dişi 40 Sprague-Dawley sıçan W'arli dört gruba bö¬lündü. 1. grup kontrol, 11. grup splenektomi, III. grup sple¬nik arter ligasyonu ve IV. grup ise splenik otot-ransplantasyon grubu olarak ayrıldı. Ameliyat sonrası 30. günde tüm deneklerin serum kompleman 3 ve 4 (C3-C4), immunglobulin M,G,A (lgM, IgG, IgA) ve fibronektin dü¬zeyleri bakıldı ve kan kültürleri alındı. Ameliyat sonrası 45. günde bütün deneklere intraperitoneal olarak "strep¬tococcus pneumoniae tip 25" enjekte edilerek .48 saat sonra aynı kan parametrelerinin düzeyleri incelendi ve hayvanlar sakrifiye edilerek akciğer doku kültürleri ya¬pıldı. Pnömokok enjeksiyonundan sonra grup II ve IV ara¬sında C3,1gM, fibronektin düzeyleri arasında istatistiksel olarak anlamlı farklılık saptandı, intraperitoneal pnö¬mokok enjeksiyonu ile oluşturulan sepsis indüksiyonu son¬rası akciğer doku kültürlerinde IH. gruhda %20, IV. grub-da ise %40 oranında üreme saptanmıştır. Grup II'ye göre grup III ve grup IV serum C3, IgM ve fibronektin dü¬zeylerinin yüksekliği istatistiksel olarak anlamlı bulundu (p<0.0I). Grup III ve Grup IV de kendi aralarında kı¬yaslandıklarında genel olarak grup Ill'deki değerlerin grup IV'e göre daha iyi olduğu ve bu farkın istatistiksel olarak anlamlı olduğu görüldü (p<0.0l).

Öz: Effectiveness of experimental splenic artery ligation and splenic autotransplantation on occurance of pneumococcal sepsis This experimental study aims to evaluate the relative ef¬fectiveness of splenic artery ligation and splenic au¬totransplantation as spleen-saving surgical procedures. Forty female Sprague-Dawley rats, weighing 162-270 g were divided into four groups: group I control, group II for splenectomy, group III for splenic artery ligation and group IV for splenic autotransplantation. Serum levels of complement 3 and 4 (C3, C4), immuno globulin M, G, A (IgM, IgG, IgA) and fibronectin were measured, and cont¬rol blood samples were cultured in all animals on the 30th postoperative day. On the 45th postoperative day Strep¬tococcus pneumoniae type 25 was injected int-raperitoneally, the same parameters tested 48th later. The animals were sacrified after blood sampling to obtain lung ■ tissue cultures. Pneumococcal injections resulted in sta¬tistically significant differences in C3, IgM, fibronectin le¬vels between the splenic artery ligation group (III) and the splenic autotransplantation group (IV). The sepsis induce by intraperitoneal Pneumococcus injection, colonization was detected in lung in group III and tissue cultures in 2 (20 %) rats in group III and 4 (40 %) rats in group IV. The elevated levels of C3, IgM and fibronectin in groups III and IV compared to group II were considered to he sta¬tistically significant (p<0.01). These results indicated a statistically significant (p<0.01) advantage of splenic ar¬tery ligation over autotransplantation.
