Bruksizm (etyoloji, fizyopatoloji, teşhis ve tedavi)

Öz:
Bruksizm; daha çok diş sıkma ve gıcırdatma olarak bilinen, çiğneme sisteminde görülen yaygın bir parafonksiyonel alışkanlıktır. Bruksizmin etyolojisi halen tartışılmaktadır ve teorilerde oklüzal, psikolojik, genetik ve stres faktörleri üzerinde durulmaktadır. Günümüzde etyolojisinin birden fazla etkenle ilişkili olduğuna dair ortak bir inanış vardır ve bruksizmin, yapısal komponentlere daha fazla stres ve ağrı davranışı ile ilişkili bir santral sinir sistemi fenomeni olduğu düşünülmektedir. Her ne kadar fizyopatolojik mekanizması tam olarak bilinmese de, çalışmalar, bruksizmin santral ve otonom sinir sisteminde oro-fasial fonksiyonlar ve uyku düzenlemesiyle ve ek olarak psiko-sosyal ve genetik faktörlerle ilgili olduğunu göstermektedir. Uyku bruksizminin (UB) kesin teşhisi polysomnografik yöntem veya ayakta kayıt sistemleriyle elektrofizyolojik cihazlar kullanılarak yapılır. Diş hekimi için UB tanımlanması esas olarak klinik gözlemlere dayanır fakat klinik diagnozun da sınırlamaları vardır. Bruksizmi daimi olarak elimine eden bir tedavi yöntemi henüz bulunmamaktadır. Her ne kadar oklüzal, ısırma plakları ile bruksizmin zararlı etkileri önlense de bu, hastalar için iyileştirici bir yöntem değildir ve oklüzal tedavi bitirildiğinde uzun dönemde bruksizm tekrarlar. Bruksizmin tedavisi bilinmediği için klinisyenler ilk aşamada daima reversibl konservatif tedavilere baş vurmalıdırlar.

Bruxism (etiology, pathophysiology, diagnosis and treatment)

Öz:
The most common tooth-to-tooth habits are clenching and grinding, i.e. bruxism. The etiology of bruxism has also been controversial and theories have invoked occlusal, psychological, genetic, and stress factors. Currently, there is consensus about the multifactorial nature of its etiology, and bruxism is thought to be a central nervous system phenomenon related to stress and pain behavior more than to structural components. Although the exact pathophysiologic mechanisms remain to be elucidated, studies suggest that sleep bruxism (SB) is influenced by central and autonomic cardiac nervous system processes related to oromotor functions and sleep-waking regulation, in addition to psychosocial and genetic factors. Definitive diagnosis of SB is made using electrophysiologic tools such as polysomnographic or ambulatory recording systems. For dentists, recognition of SB is based mainly on the clinical observations, but chairside diagnosis has limitations. Currently there is no known treatment method for permanently eliminating bruxism. Although occlusal appliances guard against the harmful effects, they do not cure a patient ofbruxing. In most instances when long-term occlusal appliance therapy is terminated, the bruxism returns. Since a cure for bruxism is unknown, the clinician should always first attempt conservative reversible therapy.
